The Cardihab App is a digital health platform that delivers tailored Cardiac Rehabilitation (CR) care plans for patients who have had a cardiac event, and chronic care plans for primary and/or secondary prevention. It is delivered via Software-as-a-Service (SaaS) and is intended for clinicians to use with patients through remote, hybrid and traditional models of care.
