The Cardihab App is a digital health platform that delivers tailored Cardiac Rehabilitation (CR) care plans for patients who have had a cardiac event, and chronic care plans for primary and/or secondary prevention. It is delivered via Software-as-a-Service (SaaS) and is intended for clinicians to use with patients through remote, hybrid and traditional models of care. The platform is used in conjunction with, and under the supervision of, qualified clinicians and is not a consumer only platform.
