Ⓡ GLUMET -FORTE METFORMIN HCL 850MG TABLET 850 MG _SURGERY_ Metformin must be discontinued at the time of surgery under general, spinal or epidural anaesthesia. Therapy may be restarted no earlier than 48 hours following surgery or resump- tion of oral nutrition and provided that renal function has been re-evaluated and found to be stable. _CARDIAC FUNCTION_ Patients with heart failure are more at risk of hypoxia and renal insufficiency. In patients with stable chronic heart failure, metformin may be used with a regular monitoring of cardiac and renal function. For patients with acute and unstable heart failure, metformin is contraindicated. _HEPATIC IMPAIRMENT_ Patients with hepatic impairment have developed cases of metformin-associated lactic acidosis. This may due to impaired lactate clearance resulting in higher lactate blood levels. Therefore, avoid use of metformin in patients with clinical or laboratory evidence of hepatic disease. _VITAMIN B_ _12_ _ DEFICIENCY_ A decrease to subnormal levels of previously normal serum vitamin B 12 levels was observed in patients. Such decrease, possibly due to interference with B 12 absorption from the B 12 -intrinsic factor complex, may be associated with anaemia but appears to be rapidly reversible with discontinuation of metformin or vitamin B 12 supplementation. Certain individuals (those with inadequate vitamin B 12 or calcium intake or absorption) appear to be predisposed to developing subnormal vitamin B 12 levels. Measure haematologic parameters on an annual basis and vitamin B 12 at 2 to 3 year intervals in patients on metformin and manage any abnormalities. _PAEDIATRIC POPULATION_ The diagnosis of type 2 diabetes mellitus should be confirmed before treatment with metformin is initiated. No effect of metformin on growth and puberty has been detected but no long-term data on these specific points are available. Therefore, a careful follow-up of the effect of metformin on these parameters in metformin-treated children, especially prepubes- cent children, is reco Đọc toàn bộ tài liệu
