Your doctor can tell you if you are at high risk for such events. 2. HOW TELMISARTAN TABLETS WORKS Telmisartan belongs to a class of medicines known as angiotensin II receptor antagonists. Angiotensin II is a substance produced in your body which causes your blood vessels to narrow, thus increasing your blood pressure. Telmisartan blocks the effect of angiotensin II so that the blood vessels relax, and your blood pressure is lowered. 3. BEFORE YOU USE TELMISARTAN TABLETS _-When you must not use it _ Do not take Telmisartan • if you are allergic to telmisartan or any other ingredients of this medicine • if you are more than 3 months pregnant. • if you have severe liver problems such as cholestasis or biliary obstruction (problems with drainage of the bile from the liver and gall bladder) or any other severe liver disease. • if you have diabetes or impaired kidney function and you are treated with a blood pressure lowering medicine containing aliskiren. PROPERTIES: Telmisartan belongs to a group of medicines called angiotensin II receptor antagonists. It blocks the effect of angiotensin II so that the blood vessels relax, and your blood pressure is lowered. Telmisartan is 4’-[[4-Methyl-6-(1- methyl-1H-benzimidazol-2-yl)-2-propyl-1H-benzimidazol-1-yl] methyl] biphenyl-2-carboxylic acid. The empirical formula of Telmisartan is C 33 H 30 N 4 O 2 and its molecular weight is 514.6. Its structural formula is CLINICAL PHARMACOLOGY: PHARMACODYNAMICS AND MECHANISM OF ACTION: Pharmacotherapeutic group: Angiotensin II receptor (type AT1) antagonist, ATC Code : C09CA07 Telmisartan blocks the vasoconstrictor and aldosterone secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues, such as vascular smooth muscle and the adrenal gland. Its action is therefore independent of the pathways for angiotensin II synthesis. Telmisartan has much greater affinity ( > 3,000 fold) for the AT1 receptor than for the AT2 receptor. Blockade of the renin-angiotensin system with ACE inhibitors, which inhibit the biosynthesis of angiotensin II from angiotensin I, is widely used in the treatment of hypertension. ACE inhibitors also inhibit the degradation of bradykinin, a reaction also catalyzed by ACE. Because telmisartan does not inhibit ACE (kininase II Przeczytaj cały dokument
