WHAT VIAGRIL IS USED FOR
VIAGRIL is indicated for
the
treatment
of
erectile
dysfunction sometimes
known
as
impotence,
which
is
the
inability to achieve or maintain
a penile erection sufficient
for
satisfactory
sexual
performance.
VIAGRIL
will
only help to get
an erection if
you
are
sexually
stimulated.
You should not take VIAGRIL
if you are women.
2.
HOW VIAGRIL WORKS
VIAGRIL belongs to a group of
medicines
called
phosphodiesterase
type
5
inhibitors. It works by helping to
relax the blood vessels in your
penis,
allowing blood to flow
into your
penis
when you get
sexually excited.
_What VIAGRIL does not do_

It
does
not
cure
erectile
dysfunction.
It
is a treatment
for erectile dysfunction.

VIAGRIL does not
protect
you
or
your
partner
from sexually
transmitted disease.

VIAGRIL is not a hormone or an
aphrodisiac
(a
substance
that
stimulates sexual desire_)_.

BEFORE YOU TAKE VIAGRIL
_When you must not take it_

If you are allergic to sildenafil
or
any
other
ingredients
of
VIAGRIL.

If you have severe heart
or liver
problem.

If
you
take
VIAGRIL
while
taking
nitrates,
the
combined
effect
of
the VIAGRIL and the
nitrates
can
lower
your
blood
pressure to such an extent that it
could
prove
fatal.
Never
take
VIAGRIL at
the same time as
taking nitrates.

If
you
have
ever
had loss
of
vision
caused
by
non-arteritic
anterior
ischaemic
optic
neuropathy (NAION),
a cause of
decreased
vision
or
loss
of
vision.
_Before you start to take it._
Inform your doctor if you have

Sickle
cell
anemia
(an
abnormality of
red blood cells),
leukemia,
multiple
myeloma
(cancer of bone marrow).


DESCRIPTION:
MAXEFIL 100MG TABLET: A blue coloured diamond shaped film coated
tablet with score on one

MAXEFIL 50MG TABLET: A blue coloured diamond shaped film coated tablet
with score on one
side
COMPOSITION: Each film coated tablet contains 50 or 100mg Sildenafil
(as the citrate salt).
PHARMACODYNAMIC: Sildenafil is a selective inhibitor of cyclic
guanosine monophosphate
(cGMP)- specific phosphodiesterase type 5 (PDE5). It is used as an
oral therapy for erectile
dysfunction.
The
mechanism
of
erection
involves
release
of
nitric
oxide
(NO)
in
the
corpus
cavernosum during sexual stimulation. Nitric Oxide activates the
enzymes guanylate cyclase. This
will result in increased levels of cGMP which produces smooth muscle
relaxation in the corpus
cavernosum and allowing inflow of blood. Sildenafil enhances the
effect of nitric oxide by inhibiting
phosphodiesterase type 5 (PDE5), which is responsible for degradation
of cGMP in the corpus
cavernosum. This results in smooth muscle relaxation and inflow of
blood to the corpus cavernosum.
It has no effect in the absence of sexual stimulation.
PHARMACOKINETICS: _ Absorption_: Sildenafil is rapidly absorbed after
oral administration.
Nonetheless, only about 40% of a dose reaches systemic circulation
unchanged. Within 30 – 120
minutes peak plasma concentration usually will be attained. High fat
meals delays GI absorption,
peak plasma concentrations are reduced by about 30% and time to peak
plasma concentration
delayed about 60min. _ Distribution_: N-desmethyl metabolite and
Sildenafil are bound to plasma
proteins. Protein binding is independent of total drug concentration.
_ Metabolism_: It undergoes
extensive metabolism in GI mucosa during absorption and on first pass
through liver. Sildenafil is
metabolized in the liver principally by CYP3A4 and to lesser extent by
CYP2C9.The major
circulating metabolites results from N-demethylation of sildenafil,
and itself further metabolized.
_Elimination_: Sildenafil is excreted as metabolites mainly in the
faece
