WHAT CARDIL IS USED FOR
Cardil Tablets contain diltiazem
hydrochloride, which belongs to a
group of medicines called calcium
channel blockers. Cardil Tablets are
used in the treatment of:

mild to moderate high blood
pressure

angina pectoris (a condition
which causes chest pain
sometimes on exertion)
HOW CARDIL WORKS
In high blood pressure, diltiazem
hydrochloride works by widening the
blood vessels. This creates less
resistance to the blood flow, and
results in lower blood pressure, which
in turn reduces the strain on your
heart.
In angina, diltiazem hydrochloride
works by opening up the arteries
supplying the heart muscle and this
allows more blood and oxygen to
reach the muscle, decreasing the
chances of angina ( chest pains )
occurring when extra strain is placed
upon the heart.
BEFORE YOU USE
_- When you must not use it _
Do not take Cardil Tablets if you:

are allergic (hypersensitive)
to diltiazem hydrochloride or
any of the other ingredients
of Cardil Tablets (see
Section 8 Product
Description)

have a heart condition which
results in an abnormal heart
beat causing palpitations or
fainting (sick sinus
syndrome or second or third
degree AV block) and do not
have a functioning
pacemaker

have a heart condition called
Wolff-Parkinson-White
syndrome and a rapid,
irregular heart beat (atrial
fibrillation/flutter)

have a very slow pulse – less
than 40 beats per minute

have heart failure with poor
blood flow (causing
breathlessness, tiredness and
ankle swelling)

have recently had a heart
attack

have very low blood
pressure

are a woman of child bearing

CARDIL 120 mg controlled release tablet
PRESENTATION
Cardil 120 mg controlled release tablets
are white, film-coated, capsule-shaped
and scored. Code DL 120. Each tablet
contains Diltiazem hydrochloride 120 mg
in a controlled-release formulation.
PHARMACODYNAMIC PROPERTIES
Pharmacotherapeutic group:
Benzothiazepine derivatives. ATC-code:
C08DB01.
Diltiazem is a benzothiazepine derivative
that effectively blocks the slow calcium
channels (L channels) of vascular smooth
muscle and cardiac muscle cells. Slow
calcium channels play an important role
especially in the regulation of sinoatrial
and atrioventricular node function in the
cardiac muscle cells.
Diltiazem possesses both peripheral
and coronary vasodilator properties.
However, diltiazem – induced fall in blood
pressure is not commonly followed by
reflectory tachycardia which is probably
due to the depressive effect of the drug
on the stimulation of sinoatrial node
function. Diltiazem slows atrioventricular
conduction. It also has a weak negative
inotropic effect on the heart. Diltiazem
improves relaxation of the cardiac muscle
and diastolic function which together
with decreased afterload improves the left
ventricular function. In spite of coronary
dilatation the total flow in healthy
coronary arteries does not usually change,
but some improvement in the circulation
of contracted arteries has been observed.
Diltiazem relaxes smooth muscle also
elsewhere in the body, e.g. the lower
oesophageal sphincter muscle. Diltiazem
has not been shown to have effect on
electrolyte-, lipid- or glucose balance in
healthy or diabetic persons.
Pharmacokinetic properties
Diltiazem is completely absorbed after
oral administration, but owing to its first-
pass metabolism in the liver, the absolute
bioavailability of diltiazem hydrochloride
is only about 40% (with interindividual
variability ranging from 24 to 74%).
The bioavailability is independent of
formulation or therapeutic dose. A diet
