paraldehyde, thiopentone sodium, and chlorpromazine have also been recommended. Severe hypotension may require fluid replacement, and assisted respiration may be necessary. Forced diuresis is of little value since antihistamines are rapidly metabolized and only traces are recovered in the urine. Large doses of Ammonium Chloride may cause a profound acidosis and hypokalaemia which should be treated symptomatically. STORAGE: Keep container well closed. Store below 30 o C. Protect from light. PHARMACOLOGY: _Mechanism of action:_ Diphenhydramine is an ethanolamines antihistamine, a H1-receptor antagonist. It acts by competing with histamine for H1-receptor sites on the effectors cells, to the exclusion of agonist molecules, without itself initiating a response. This antagonism is competitive and reversible. It does not block the release of histamine. Ammonium chloride is used as an expectorant. _Pharmacokinetic:_ Diphenhydramine is readily absorbed from gastrointestinal tract and parental sites of administration. Systemic availability is affected by high first-pass metabolism. Peak plasma concentration is within 1-4 hours after oral administration. It is highly bound to plasma protein, thus widely distributed throughout the body including the CNS. It crosses the placenta and has been detected in breast milk. Most of the diphenhydramine is excreted as metabolites through urine with complete excretion within 24 hours. Ammonium chloride is absorbed from the gastrointestinal tract.
