HOW RETEN FIVE WORKS Reten Five possible work by increasing diameter of hair shaft, stimulate active growth phase and enhancing microcirculation to hair follicles. The exact way that Reten Five work is not known. BEFORE YOU USE RETEN FIVE_ _ - _When you must not use it_ • You are hypersensitive to the active ingredient or any other ingredients in the formulation. • You are under 18 years old or over 65 years old. - _Before you start to use it _ Tell your doctor if you: • Have any unusual or allergic reaction to minoxidil or other ingredients in this preparation • Have any other skin problems, an irritation, or a sunburn on the scalp • Have heart problems or high blood pressure • Have tumour of the adrenal gland • Are using other topical preparation • Are pregnant or nursing • Are HIV positive • Have kidney problems If you are not sure whether you should start taking this product, consult your doctor. - _Taking other medicines _ Tell your doctor if you are taking any other medicine, including any that you buy without a prescription. You may need to use different amounts of these medicines, or you may need to take different medicines. Your doctor will advise you. PHARMACODYNAMICS Minoxidil stimulates hair growth in alopecia androgenetica males and females when applied topically. The mechanism by which minoxidil stimulates hair growth and reverses the hair loss process is not fully established, but possible mechanisms may include the following: - increasing the diameter of the hair shaft and stimulating of hair follicles, - stimulating anagen (active) growth, - prolonging the anagen phase - stimulation of telogen phase (resting hair follicles) into anagen phase (active growth) - enhancing microcirculation to hair follicles (as peripheral vasodilator). The Vascular Endothelial Growth Factor (VEGF) is stimulated by minoxidil and VEGF is presumably responsible for the increased capillary fenestration, indicative of a high metabolic activity, observed during the anagen phase. Hair growth can be expected approximately after 4 or more months of application of use and is variable among patients. Upon discontinuation of treatment with minoxidil, new hair growth stops and pre-treatment appearance may return within 3 – 4 months. PHARMACOKINETICS Following topical application, minoxidil is poorly absorbed from normal intact skin, with an average of 1.7% (range 0.3 – 4.5%) of the total applied dose ultimately reaching the systemic circulation. Three major factors that can increase the absorption of minoxidil are increased of the dose applied, increased of the dosing frequency, and decreased of the barrier function of the stratum corneum. Serum minoxidil level and systemic effects resulting from the administration of minoxidil are governed by the drug’s absorption rate through the skin. Minoxidil is not bound to plasma protein and does not cross blood brain barrier. Minoxidil and its metabolites are haemodialysable, and excreted principally in the urine within 4 days.
