EMEND ® I.V. _Fosaprepitant_ CONSUMER MEDICINE INFORMATION WHAT IS IN THIS LEAFLET This leaflet answers some common questions about EMEND I.V.. It does not contain all the available information. IS USED FOR CHEMOTHERAPY INDUCED NAUSEA AND VOMITING EMEND I.V., in combination with other medicines, is used to prevent • nausea (feeling sick) and vomiting associated with cancer chemotherapy. EMEND I.V. belongs to a group of medicines called neurokinin 1 (NK1) receptor antagonists. It works by blocking the actions of substances in your brain, called substance P neurokinins, that cause nausea and vomiting. Your doctor may have prescribed EMEND I.V. for another reason. Ask your doctor if you have any questions about why EMEND I.V. has been prescribed for you. The safety and effectiveness of EMEND I.V. in children and teenagers under the age of 18 years have not been established. EMEND I.V. is not addictive. BEFORE YOU ARE GIVEN EMEND I.V. _WHEN YOU MUST NOT BE GIVEN_ _IT_ DO NOT USE EMEND I.V. IF YOU HAVE AN ALLERGY TO EMEND I.V., APREPITANT OR ANY OF THE INGREDIENTS LISTED AT THE END OF THIS LEAFLET. PRODUCT INFORMATION EMEND I.V. ( fosaprepitant dimeglumine, MSD ) DESCRIPTION Fosaprepitant dimeglumine is a prodrug of aprepitant and is chemically described as 1-Deoxy-1-(methylamino)-D-glucitol [3-[[(2_R_,3_S_)-2-[(1_R_)-1-[3,5- bis(trifluoromethyl)phenyl]ethoxy]-3-(4-fluorophenyl)-4-morpholinyl]methyl]-2,5- dihydro-5-oxo-1_H_-1,2,4-triazol-1-yl]phosphonate (2:1) (salt). Aprepitant is a structurally novel substance P neurokinin 1 (NK 1 ) receptor antagonist, chemically described as 5-[[(2_R_,3_S_)-2-[(1_R_)-1-[3,5-bis(trifluoromethyl)phenyl]ethoxy]- 3-(4-fluorophenyl)-4-morpholinyl]methyl]-1,2-dihydro-3_H_-1,2,4-triazol-3-one. Its empirical formula is C 23 H 21 F 7 N 4 O 3 , and its structural formula is: Aprepitant is a white to off-white crystalline solid, with a molecular weight of 534.43. It is practically insoluble in water. Aprepitant is sparingly soluble in ethanol and isopropyl acetate and slightly soluble in acetonitrile. EMEND I.V. is available as a 150 mg and 115 mg I.V. for infusion. Each vial of EMEND I.V. 115 mg for intravenous administration contains 188 mg of fosaprepitant dimeglumine equivalent to 115mg of Fosaprepitant free acid.
