Date of revision What DYNAMET TABLET is used for • Duodenal and gastric ulcers. • Reflux oesophagitis (backflow of stomach content into the esophagus (food pipe). • Abnormal excessive secretion of gastric acid such as Zollinger-Ellison syndrome. • Pancreatic insufficiency. How DYNAMET TABLET works Cimetidine inhibits stimulation and secretion of gastric acid and reduces output of pepsin enzymes. Also, it affects on the reduction secretion of body fluids. Cimetidine also provide protection to cells against harmful agents and has a beneficial effect in maintaining the gastric mucosal lining. Before you use DYNAMET TABLET - When you must not use it Do not use this medicine if: • You have lack or complete absence of gastric juice. • You have abnormal growth and proliferation of abnormal cells. • You are pregnant. Pregnancy and lactation Do not take DYNAMET TABLET if you are pregnant, trying to get pregnant or think you may be pregnant. Cimetidine was detected in the breast milk. Do not take DYNAMET TABLET if you are breast-feeding. Ask your doctor or pharmacist for advice before taking any medicine. - Before you start use it Tell your doctor or pharmacist if you have impaired kidney function and possibility of cancer should be excluded before starting the treatment. - Taking other medicines Tell your doctor if you are taking: • Lignocaine (local anaesthetic); • Phenytoin (prevent seizure); • Theophylline (prevent and treat asthma); • Warfarin (prevent blood clotting); and any other medicines, including any that you buy without a prescription from a pharmacy, supermarket or health food shop. It competitively inhibits the action of histamine at the histamine H2-receptor of the parietal cells and thus is a histamine H2-receptor antagonist. In addition to its antisecretory effects, Cimetidine also has cytoprotective properties and therefore has a beneficial effect in maintaining the integrity of the gastric mucosal barrier. PHARMACOKINETICS: Cimetidine is readily absorbed from the gastrointestinal tract and peak plasma concentrations are obtained about an hour after administration on an empty stomach and about 2 hours after administration with food. The bioavailability of Cimetidine following oral administration is about 60% to 70% compared to an intravenous dose due to first-pass metabolism. The elimination half-life from plasma is around 2 hours and Cimetidine is weakly bound to about 20% to plasma proteins. Cimetidine is partially metabolised in the liver to the sulphoxide and to hydroxymethylcimetidine but most is excreted unchanged in the urine. Cimetidine crosses the placental barrier and is excreted into breast milk where concentrations are reported to be higher than those in plasma. It does not readily cross the blood brain barrier. INDICATIONS: Duodenal and gastric ulcers. Reflux oesophagitis. Pathological hypersecretory conditions such as Zollinger-Ellison syndrome. Pancreatic insufficiency.
