As an adjunct to diet and exercise for the improvement of glycemic control in patients with type 2 diabetes mellitus who are already treated with pioglitazone and a sulfonylurea and whose diabetes is not adequately controlled with glimepiride alone or those patients who have initially responded to pioglitazone alone and require additional glycemic control.

DESCRIPTION Pioglitazone+Glimepiride (Zoliget ) combines two antihyperglycemic agents to improve glycemic control with type 2 diabetes: pioglitazone hydrochloride, a member of the thiazolidinedione class, and glimepiride, a member of the sulfonylurea class. Chemically pioglitazone is [(±)-5-[[4-[2-(5-ethyl-2-pyridinyl) ethoxy] phenyl] methyl]- 2, 4-] thiazolidinedione monohydrochloride. The molecular formula is C 19 H 20 N 2 O 3 S.HCl. The structural formula is: Chemically glimepiride is 1-[[p-[2-(3-ethyl-4-methyl-2-oxo-3-pyrroline-1- carboxamido) ethyl] phenyl]-sulphonyl]-3-(trans-4-methylcyclohexyl) urea. Pioglitazone+Glimepiride (Zoliget ) Tablets 30mg+2mg Each tablet contains: Pioglitazone (as HCl) … 30mg Glimepiride… 2mg CLINICAL PHARMACOLOGY MECHANISM OF ACTION Pioglitazone+Glimepiride (Zoliget ) is a combination of two antihyperglycemic agents to improve glycemic control in patients with type 2 diabetes. _Pioglitazone:_ Pioglitazone HCl is a thiazolidinedione antidiabetic agent that depends on the presence of insulin for its mechanism of action. Pioglitazone HCl decreases insulin resistance in the periphery and in the liver resulting in increased insulin- dependent glucose disposal and decreased hepatic glucose output. Unlike sulfonylureas, pioglitazone is not an insulin secretagogue. Pioglitazone HCl is a potent and highly selective agonist for peroxisome proliferator-activated receptor- gamma (PPAR ). PPAR receptors are found in tissues important for insulin action such as adipose tissue, skeletal muscle, and liver.
