PERMIT FOR SUPPLY OF SPECIFIC BATCHES OF A REGISTERED VETERINARY
CHEMICAL WITH AN UNAPPROVED LABEL AND IN AN UNAPPROVED PACK SIZE
PERMIT 81562
This permit is issued to the Permit Holder under Section 112 of the
Agricultural and
Veterinary Chemicals Code, scheduled to the _ Agricultural and
Veterinary Chemicals Code _
_Act 1994_ (the Agvet Code) of the jurisdictions set out below. This
permit allows a person, as
stipulated below, to supply the product in the manner specified in
this permit in the designated
jurisdictions. If this permit were not issued, supply of the product
as specified below would
constitute an offence under section 81 and 79 of the Agvet Code.
THIS PERMIT IS IN FORCE FROM 19 AUGUST 2015 TO 30 NOVEMBER 2017

CONDITIONS
SUPPLY
1.
The supplier must supply The Product in compliance with the conditions
of product
registration, except for pack size and label requirements as detailed
in condition 2 and 3 of
this permit.
2.
The Product may be supplied in a pack size of 100 syringes.
3.
The Product is permitted for supply with:
a.
the New Zealand-registered label attached to the immediate packaging
(the syringe)
and
b.
an oversticker covering part of the outer packaging (the tub) that
states the registered
label particulars (RLPs) for the Australian registered product (65565)
and reference
to permit number PER81562.
