Date of revision WHAT GIAGRA IS USED FOR GIAGRA is indicated for the treatment of erectile dysfunction, which is the inability to achieve or maintain a penile erection sufficient for satisfactory sexual performance. In order for GIAGRA to be effective, sexual stimulation is required. HOW GIAGRA WORKS It works by helping to relax the blood vessels in your penis, allowing blood to flow into your penis when you get sexually excited. GIAGRA will only help you to get an erection if you are sexually stimulated. You should not take GIAGRA if you do not have erectile dysfunction. BEFORE YOU USE GIAGRA - When you must not use it DO NOT TAKE GIAGRA • If you are taking medicines called nitrates, as the combination may Cause a potentially dangerous decrease in your blood pressure. Tell your doctor if you are taking any of these medicines which are often given for relief of angina pectoris (or “chest pain”). If you are not certain, ask your doctor or pharmacist. • If you are using any of the drugs known as nitric oxide donors such as amyl nitrite (“poppers”) as the combination may also lead to a potentially dangerous decrease in your blood pressure. • If you are allergic (hypersensitive) to GIAGRA or any of the other ingredients of GIAGRA. • If you have a severe heart or liver problem. • If you have recently had a stroke or a heart attack, or if you have low blood pressure. • If you have certain rare inherited eye diseases (such as retinitis pigmentosa). GIAGRA 25, 50 & 100 Sildenafil Citrate Tablets 25, 50 & 100 mg GIAGRA 25 (Sildenafil Citrate Tablets 25 mg) Description: Blue colored round, biconvex, film coated tablets, debossed with 124 on one side and J on the other side. Each film coated tablets contains Sildenafil Citrate 35.12 mg is equivalent to Sildenafil 25 mg. GIAGRA 50 (Sildenafil Citrate Tablets 50 mg) Description: Blue colored round, biconvex, scored film coated tablets, debossed with 125 on one side and J on the other side with score line. Each film coated tablets contains Sildenafil Citrate 70.24 mg is equivalent to Sildenafil 50 mg. GIAGRA 100 (Sildenafil Citrate Tablets 100 mg) Description: Blue colored round, biconvex, scored film coated tablets, debossed with 126 on one side and J on the other side with score line. Each film coated tablets contains Sildenafil Citrate 140.48 mg is equivalent to Sildenafil 100 mg DOSAGE FORMS: Tablet PHARMACODYNAMICS Mechanism of action The physiologic mechanism of erection of the penis involves release of nitric oxide (NO) in the corpus cavernosum during sexual stimulation. NO then activates the enzyme guanylate cyclase, which results in increased levels of cyclic guanosine monophosphate (cGMP), producing smooth muscle relaxation in the corpus cavernosum and allowing inflow of blood. Sildenafil has no direct relaxant effect on isolated human corpus cavernosum, but enhances the effect of nitric oxide (NO) by inhibiting phosphodiesterase type 5 (PDE5), which is responsible for degradation of cGMP in the corpus cavernosum. When sexual stimulation causes local release of NO, inhibition of PDE5 by sildenafil causes increased levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood to the corpus cavernosum. Sildenafil at recommended doses has no effect in the absence of sexual stimulation. _In vitro_, sildenafil is found to be selective for PDE5.
