PEN-STREP 20/20 INJECTABLE SUSPENSION 1. NAME PEN-STREP 20/20 2. COMPOSITION Benzylpenicillin procaine 200.000 I.U. - Dihydrostreptomycin sulphate (250 mg) equiv. to dihdyrostreptomycin 200 mg EXCIPIENTS: Sodium citrate 10 mg - Sodium methylparahydroxybenzoate 1.14 mg - Povidone 5 mg - Sodium formaldehyde sulfoxylate 2 mg - Water for injections up to 1 ml. 3. PHARMACEUTICAL FORM Injectable suspension for intramuscular injection. 4. PHARMACOLOGICAL PROPERTIES PEN-STREP 20/20 is a combination of the procaine salt of benzyl penicillin and dihydrostreptomycin (as sulphate). * _Pharmacological groups:_ procaine benzyl penicillin is a beta-lactam antibiotic and dihydrostreptomycin belongs to the amino glycosides. * _Mode of action:_ - Benzyl penicillin interferes with the enzymatic development of the bacterial cell wal of Gram-positive bacteria. The inability of benzyl penicillin to affect Gram-negative bacteria can be explained by the structure of the Gram-negative cell wall: the extra layer in the Gram-negative cell wall consists of endotoxin (lipopolysaccharide) with benzyl penicillin cannot penetrate. Penicillins are bactericidal at high concentrations (from 0.05 IU/ml), bacteriostatic at low concentrations. Antibacterial action of benzyl penicillin is highest during the period of greates bacterial multiplication (acute infections). - Dihydrostreptomycin binds specifically to a protein in the bacterial 30S ribosomal subunit. In susceptible bacteria this binding causes disruption of the ribosomal subunit and "misreading" of the information on the messenger RNA. This misreading leads to the synthesis by the bacterium of what are called "nonsense" proteins which do not fit anywhere in the bacterium. Dihydrostreptomycin acts bactericidal and is most effective against Gram-negative bacteria such as _Pasteurella, Brucella, _ _Haemophilus, Salmonella, Shigella, Klebsiella _and _Mycobacterium_. The combination of both antibiotics has a broad-spectrum action against both Gram-negative and Gram-positive organisms. SUMMARY OF Прочитать полный документ
