WHAT VOKER FILM COATED
TABLET
IS USED FOR
It is used to treat ulcers (sores on the lining of
the
stomach
or
small
intestine),
reflux
oesophagitis
(inflammation
of
oesophagus
accompanied by the regurgitating of stomach
acid)
and
Zollinger-Ellison
Syndrome
(condition where the stomach produces too
much acid).
HOW VOKER FILM COATED
TABLET
WORKS
Famotidine
is
known
as
an
H
2
histamine
blocker. It works by reducing the amount of
acid in your stomach.
BEFORE YOU USE VOKER FILM COATED
TABLET
_-When you must not use it _
VOKER FILM COATED TABLET should
not be used if you are sensitive to the
ingredients
.
Symptomatic
response
to
therapy
with
Famotidine does not preclude the presence of
gastric cancer.
_-Before you start to use it _
Tell your doctor if you are allergic to any
other medicines or you have any other health
related problems.
_-Taking other medicines _
Seek for your doctor or pharmacist advice if
you intended to use VOKER FILM COATED
TABLET
together
with
other
medication.
Besides, you must also inform them about all
the medicines you are taking, including those
without a prescription.
HOW TO USE VOKER FILM COATED
TABLET
_-How much to use _
Duodenal
and
benign
gastric
ulcer:
Initial daily dosage is 40mg at bedtime
for
up
to
4~
8
weeks,
maintenance:
20mg at night time.
Zollinger-Ellison
Syndrome
initially
20mg
every
6
hours;
maximum
of
800mg daily. Reflux esophagitis: 20 to
40mg twice daily for 6 to 12 weeks.
Dosage
adjustment
is
required
for
people with moderate to severe kidney
insufficiency.

PHARMACOLOGY (SUMMARY OF PHARMACODYNAMIC AND PHARMACOKINETIC):
1.
Clarithromycin
exerts
its
antibacterial
activity
by
inhibiting
the
synthesis
of
proteins, by means of a link with the 50S sub-unit of the cellular
ribosome.
Clarithromycin has demonstrated excellent _in vitro_ activity against
both standard
strains of bacteria and clinical isolates. It is highly potent against
a wide variety of
aerobic and anaerobic gram-positive and gram-negative organisms.
2.
ABSORPTION
Well absorbed from the gastrointestinal tract; stable in gastric acid;
food delays
the rate, but not the extent, of absorption; bioavailability is
approximately 55% in
healthy volunteers.
3.
DISTRIBUTION
Widely distributed into tissues and fluids; high concentrations found
in nasal
mucosa, tonsils, and lungs; concentrations in tissues are higher than
those in
serum because of high intracellular concentrations; readily enters
leucocytes and
macrophages.
Volume of distribution: 243-266 liters.

PROTEIN BINDING
High (65-75%).
5.
BIOTRANSFORMATION
Hepatically metabolized via three main pathways, demethylation,
hydroxylation,
and hydrolysis, to eight metabolites. One metabolite,
14-hydroxyclarithromycin,
has _in vitro _antimicrobial activity comparable to that of
clarithromycin and may act
synergistically with clarithromycin against _ Haemophilus influenzae_.
Saturation of
metabolism involves the demethylation and hydroxylation pathways, and
accounts
for an increase in serum half-life.
6.
HALF-LIFE
Clarithromycin:
250mg every 12 hours – 3 to 4 hours.
500mg every 12 hours – 5 to 7 hours.
14-hydroxyclarithromycin:
250mg every 12 hours – 5 to 6 hours.
