Permit PER13713 Permit Version 2 Page 1 of 2 PERMIT TO ALLOW THE MINOR OFF-LABEL USE OF A REGISTERED VETERINARY CHEMICAL PRODUCT PERMIT NUMBER –PER13713 This permit is issued to the Permit Holder in response to an application granted by the APVMA under section 112 of the Agvet Codes of the jurisdictions set out below. This permit allows a person, as stipulated below, to use the product in the manner specified in this permit in the designated jurisdictions. This permit also allows any person to claim that the product can be used in the manner specified in this permit. If this permit were not issued, supply and use of the product, as specified below, would constitute an offence under section 79 of the Agvet Code. THIS PERMIT IS IN FORCE FROM 04 SEPTEMBER 2012 TO 31 DECEMBER 2017. PERMIT HOLDER: BIOSECURITY QUEENSLAND Department of Agiculture, Fisheries and Forestry, Level 5, Primary Industries Building, 80 Ann Street BRISBANE QLD 4001 PERSONS WHO CAN USE THE PRODUCTS UNDER THIS PERMIT: Registered veterinary surgeons, officers of Biosecurity Queensland and any other person, acting under the direct supervision of a registered veterinary surgeon. DOSE AND ADMINISTRATION: As directed by a registered veterinary surgeon. CONDITIONS OF USE PRODUCTS TO BE USED: Any veterinary chemical product containing 100 mg/mL XYLAZINE as their only active constituent and is registered for use in horses and/or cattle. CRITICAL USE COMMENTS: 1. This Permit comes into force after notification of an emergency disease by the State/Territory Chief Veterinary Officer(s) to the Consultative Committee on Exotic Animal Diseases (CCEAD) and following invocation of the CCEAD Response Agreement under Permit PER13713 Permit Version 2 Page 2 of 2 AUSVETPLAN; or, following the declaration of an emergency disease incident by the Queensland Chief Veterinary Officer using powers provided in the legislation of that State or Territory. 2.
