It contains the active ingredient diltiazem hydrochloride. It is used to treat: • hypertension (high blood pressure) • angina. HYPERTENSION: Everyone has blood pressure. This pressure helps move your blood around your body. Your blood pressure may be different at different times of the day, depending on how busy or worried you are. You have hypertension (high blood pressure) when your blood pressure stays higher than is needed, even when you are calm and relaxed. There are usually no symptoms of hypertension. The only way of knowing that you have hypertension is to have your blood pressure checked on a regular basis. You may feel fine and have no symptoms, but if high blood pressure is not treated, it can lead to serious health problems. Chemically diltiazem hydrochloride is the hydrochloride salt of (2S,3S)-5-(2-dimethylaminoethyl) 2,3,4,5-tetrahydro-2- (4-methoxyphenyl)-4-oxo-1,5,-benzothiazepin-3-yl acetate. N S NMe 2 H OAc OMe O H 1 HCl Molecular Formula: C 22 H 26 N 2 O 4 S.HCl CAS Number: 33286-22-5 Diltiazem hydrochloride has a molecular weight of 451. DESCRIPTION Diltiazem hydrochloride is a calcium ion influx inhibitor (slow channel blocker or calcium antagonist). Diltiazem hydrochloride is a white to off-white crystalline powder with a bitter taste. It is soluble in water, methanol, and chloroform. PHARMACOLOGY PHARMACODYNAMICS The therapeutic benefits achieved with diltiazem are believed to be related to its ability to inhibit the influx of calcium ions during membrane depolarisation of cardiac and vascular smooth muscle. Mechanisms of Action Although precise mechanisms of its antianginal actions are still being delineated, diltiazem is believed to act in the following ways: 1. _Angina due to Coronary Artery Spasm:_ Diltiazem has bee shown to be a potent dilator of coronary arteries (both epicardial and subendocardial). Spontaneous and ergometrine-induced coronary artery spasm are inhibited by diltiazem. 2. _Exertional Angina:_ Diltiazem has been shown to produce increases in exercise tolerance, probably due to its ability to reduce myocardial oxygen demand and increase oxygen supply.
