Date of revision WHAT PENICILLIN V TABLET IS USED FOR This medicine can be used to treat the infections of the: • nose, throat or ear • lungs • urinary tract Penicillin is also indicated for prevention of rheumatic fever (an inflammatory disease which can cause damage to the skin, joints, brain or heart) and prevention against infection after tonsils removal and tooth extraction It will not work against infections caused by viruses, such as colds. HOW PENICILLIN V TABLET WORKS It is an antibiotic that belongs to a group of medicines called penicillins. These antibiotics work by killing the bacteria that are causing the infection. BEFORE YOU USE PENICILLIN V TABLET _-When you must not use it _ Do not take PENICILLIN V TABLET if you have allergic reaction to: - • Phenoxymethylpenicillin • Any other penicillins • Any of the ingredients listed at the end of this leaflet. Do not take this medicine if you have serious acute infections. _-Before you start to use it _ Tell your doctor if you have or have had any health problems especially kidney impairment. There is possibility of cross-sensitivity with other beta-lactam antibiotics. Please tell your doctor or pharmacist if you are required to take PENICILLIN V TABLET together with other beta- lactam antibiotics. Please inform your doctor if you have previous history of hypersensitivity reactions to penicillin, cephalosporins, carbapenem, or beta-lactam antibiotics. ADVERSE EFFECTS Each tablet contains Phenoxymethylpenicillin Potassium equivalent to 125 mg of Phenoxymethylpenicillin. Phenoxymethylpenicillin is inactivated by penicillinase and other beta-lactamases. Phenoxymethylpenicillin binds to penicillin binding proteins located on the inner membrane of the bacterial cell wall. Phenoxymethylpenicillin binds to and inactivates these proteins resulting in weakening of the bacterial cell wall and lysis. Phenoxymethylpenicillin is a broad spectrum beta-lactam antibiotic with bactericidal action against Gram-positive bacteria and Gram- negative cocci. Its antimicrobial action is similar to that of benzylpenicillin. Phenoxymethylpenicillin is usually active against the following organisms: a) Gram-positive aerobes and anaerobes including: _ Bacillus anthracis, _ _Clostridium perfringens, Clostridium tetani, Corynebacterium diphtheriae, _ _Erysipelothrix rhusiopathiae, Listeria monocytogenes, Peptostreptococcus _ _spp., Streptococcus agalactiae_ (Group B), _ Streptococcus pneumoniae, _ _and Streptococcus pyogenes_ (Group A). b) Gram-negative including _Neisseria meningitidis_ and _Neisseria _ _gonorrhoeae_ PHARMACOKINETICS ABSORPTION: Phenoxymethylpenicillin is stable under acidic conditions so it can be administered by oral route. Phenoxymethylpenicillin is rapidly, but incompletely absorbed after oral administration and the absorption level is around 60%. The simultaneous administration of food slightly decreases the peak plasma concentration of phenoxymethylpenicillin, but does not appear to affect the extent of absorption. Peak plasma concentrations are reached in about 45 minutes. The peak plasma concentration increases approximately in proportion with increased doses. Peak serum concentrations of 3-6 jig per ml have been seen following dosage of 250 mg to 500 mg by mouth. DISTRIBUTION: Phenoxymethylpenicillin is widely distributed in the body tissues and fluids (volume of distribution about 0.2 1 kg-1 of body weight) and more readily penetrates inflamed tissues.
