HOW VALZAAR H WORKS VALZAAR H belongs to a class of medicines which help to control high blood pressure. VALZAAR H works by blocking the effect of angiotensin II which cause increase in blood pressure. As a result, blood vessels relax and blood pressure is lowered. BEFORE YOU USE VALZAAR H - _When you must not use it _ Do not take VALZAAR H: • If you are allergic (hypersensitive) to valsartan or hydrochlorothiazide or any of the other ingredients of VALZAAR H. • If you have severe liver disease. _Pregnancy and lactation_ Do not take VALZAAR H if you are pregnant, trying to get pregnant or think you may be pregnant. If you get pregnant while taking VALZAAR H, stop taking it immediately and contact your doctor. Do not take VALZAAR H if you are breast-feeding. Ask your doctor or pharmacist for advice before taking any medicine. - _Before you start to use it _ You must be especially careful and talk to your doctor before taking VALZAAR H if any of the following apply to you. • If you have liver disease. • If you have severe kidney disease or if you are undergoing dialysis. • If you are suffering from a narrowing of the kidney artery. • If you have recently undergone kidney transplantation (received a new kidney). • If you are being treated after a heart attack or for heart failure. DOSAGE FORM Film coated Tablets PHARMACODYNAMICS / PHARMACOKINETICS PHARMACODYNAMICS The active hormone of the Renin-Angiotensin-Aldosterone System (RAAS) is angiotensin II, which is formed from angiotensin I through ACE. Angiotensin II binds to specific receptors located in the cell membranes of various tissues. It has a wide variety of physiological effects, including in particular both direct and indirect involvement in the regulation of blood pressure. As a potent vasoconstrictor, angiotensin II exerts a direct pressor response. In addition, it promotes sodium retention and stimulation of aldosterone secretion. Valsartan is an orally active and specific angiotensin II (Ang II) receptor antagonist. It acts selectively on the AT1 receptor subtype, which is responsible for the known actions of angiotensin II. The increased plasma levels of Ang II following ATl receptor blockade with valsartan may stimulate the unblocked AT2 receptor, which appears to counterbalance the effect of the AT1 receptor. Valsartan does not exhibit any partial agonist activity at the AT1 receptor and has much greater affinity for the AT1 receptor than for the AT2 receptor.
