DYNAPRIM SUSPENSION MAL19910828AZ DESCRIPTION: SUSPENSION Colour : White Flavour : Blended flavour of milk and vanilla CONTENT: Each 5 ml contains: Sulphamethoxazole .................. 200 mg Trimethoprim .......................... 40 mg PHARMACODYNAMICS: Sulphamethoxazole and Trimethoprim interfere with the bacterial synthesis of tetrahydrofolic acid, and essential stage in the production of thymidine, purines, and subsequently nucleic acids. Sulphamethoxazole, like the other sulphonamides, inhibits the synthesis of dihydrofolic acid from p-aminobenzoic acid. Trimethoprim inhibits the action of dihydrofolate reductase and prevents the synthesis of tetrahydrofolic acid from dihydrofolic acid; although this stage also occurs in man, Trimethoprim is much less active against the mammalian enzyme. Enhanced antibacterial activity has been reported when Sulphamethoxazole and Trimethoprim are used together in vitro although there is doubt as to whether sequential blockage of the bacterial synthetic pathway is responsible. The clinical relevance of this enhanced activity is uncertain especially in urinary-tract infections where inhibitory concentrations of Trimethoprim appear to be dominant. PHARMACOKINETICS: Trimethoprim is readily absorbed from the GIT and peak concentrations occur about 3 hours after a dose is taken. About 45% of Trimethoprim is bound to plasma proteins. Tissue concentrations are reported to be higher than serum concentrations, with particularly high concentrations occurring in the kidneys and lungs but concentrations in the cerebrospinal fluid are about one-half of those in the blood. The half-life is about 10 to 16 hours. About 40 to 50% of a dose is excreted unchanged in the urine within 24 hours, together with metabolites. Trimethoprim appears in breast milk. Sulphamethoxazole is readily absorbed from the GIT and peak plasma concentrations are reached within 4 hours. Doses of 1 g twice daily should produce blood concentrations of unconjugated Sulphamethoxazole in excess of 50 mcg per ml. About 65 Baca dokumen lengkap
