WHAT SOMAC IS USED FOR ULCERS Somac is used to treat and help heal duodenal and gastric ulcers. Depending on the position of the ulcer it is called a gastric or duodenal ulcer. A gastric ulcer occurs in the stomach. A duodenal ulcer occurs in the duodenum which is the tube leading out of the stomach. These can be caused in part by too much acid being made in the stomach. Most people who have a peptic ulcer also have bacteria called _Helicobacter pylori_ in their stomach. _SOMAC_ _®_ _ PI (Injection) _ _Page 1 of 10 _ PRODUCT INFORMATION SOMAC ® 40MG INJECTION NAME OF THE MEDICINE Active ingredient: Pantoprazole sodium Chemical Name (CAS): Sodium-[5-(difluoromethoxy)-2-[[(3,4-dimethoxy-2- pyridinyl)-methyl]-sulfinyl]-1H-benzimidazolide. Molecular formula: C 16 H 14 F 2 N 3 NaO 4 S CAS Number: 102 625-70-7 (pantoprazole) Molecular weight: 405.5 Structural formula: Na N N OCF 2 H N CH 2 S OCH 3 OCH 3 O * DESCRIPTION Pantoprazole 40 mg injection contains 42.3 mg pantoprazole sodium, 1 mg of disodium edetate, and 0.24 mg of sodium hydroxide. Pantoprazole sodium is a white to off-white amorphous hygroscopic powder. Solubility is low at neutral pH and increases with increasing pH. The 40 mg Injection contains the inactive ingredients disodium edetate and sodium hydroxide. PHARMACOLOGY PHARMACODYNAMICS Pantoprazole is a proton pump inhibitor. It inhibits specifically and dose-proportionately H+/K+-ATPase, the enzyme which is responsible for gastric acid secretion in the parietal cells of the stomach. _SOMAC_ _®_ _ PI (Injection) _ _Page 2 of 10 _ The substance is a substituted benzimidazole which accumulates in the acidic environment of the parietal cells after absorption. There, it is converted into the active form, a cyclic sulphenamide which binds to the H+/K+-ATPase, thus inhibiting the proton pump and causing potent and long-lasting suppression of basal and stimulated gastric acid secretion. As pantoprazole acts distal to the receptor level, it can influence gastric acid secretion irrespective of the nature of the stimulus (acetylcholine, histamine, gastrin).
