Metformin is used for the sort of diabetes called Type 2 diabetes or non- insulin-dependent diabetes mellitus. HOW GLUCOMIN 850 WORKS • In type 2 diabetes, there is too much sugar (glucose) in your blood. This is because your body does not make enough insulin or because it makes insulin that does not work properly. • Insulin is a hormone that allows your body tissue to take glucose from the blood and use it for energy or for storage for future use. • Metformin works by improving the sensitivity of your body to insulin. It helps your body to use glucose in the normal way again BEFORE YOU USE GLUCOMIN 850 _When you must not take it _ • you are allergic (hypersensitive) to metformin or any of the other ingredients • you have recently had a heart attack or any other heart problems • you have severe circulation problems or difficulty in breathing • you have liver or kidney problems • you have had serious problems with your diabetes in the past called diabetic ketoacidosis. When you have this you lose weight quickly, feel sick (nausea) or are sick (vomiting). • you have recently had a severe infection, injury or trauma (shock) • you are dehydrated • you are going to have an X-ray where you will be injected with a dye • you are a heavy drinker of alcohol (more than 21 units a week for a man or 14 units a week for a woman). GLUCOMIN 850MG TABLET DESCRIPTION: A white to off white oblong shaped film coated tablet. COMPOSITION: Each film coated tablet contains 850mg Metformin HCl equivalent to Metformin. PHARMACODYNAMIC: Metformin is a biguanide derivative of guanidine, used for treating type II diabetes mellitus. It has antihyperglycaemic effects, which lowers both basal and postprandial plasma glucose. It does not increase insulin secretion and hence, does not cause hypoglycaemia. Metformin acts via 3 mechanisms: reduction in hepatic glucose production, reduction in intestinal glucose absorption, and Increased insulin sensitivity (improved peripheral glucose uptake and utilization). Metformin stimulates intracellular glycogen synthesis by acting on glycogen synthase. Metformin increases the transport capacity of all types of membrane glucose transporters (GLUTs).In humans, independently of its action on glycaemia, metformin has favourable effects on lipid metabolism. PHARMACOKINETICS: _Absorption_ : After an oral dose of metformin hydrochloride tablet, maximum plasma concentration (Cmax) is reached in approximately 2.5 hours (tmax). Metformin hydrochloride is slowly and incompletely absorbed from the gastrointestinal tract. It is assumed that the pharmacokinetics of metformin absorption is non-linear. Bioavailability for oral metformin is approximately 50-60% in healthy subjects. Dose proportionally is lacking due to decreasing absorption with increasing doses. Steady state plasma concentrations for metformin are reached within 24 to 48 hours and are generally less than 1 microgram/ml at recommended doses and dosing schedules. Food decreases the extent and delays the time to achieve maximum absorption. The plasma peak concentration may be 40% lower with food administration. _Distribution_ : Plasma protein-binding is negligible. Metformin crosses the placenta and is distributed into breast milk in small amounts.
