to do so by the doctor.

Do not use the medicine after the expiry date (exp. date) that

appears on the package and bottle. The expiry date refers to the

last day of that month.

Storage conditions: Store in a cool place, below 25°C, in the

original package in a standing position.

After first opening, it can be used for 12 months.

In addition to the active ingredient, Avilac Syrup may contain small

amounts of sugars, such as:

Tagatose, fructose, galactose, epilactose, lactose.

If you have been told by a doctor that you have an intolerance

to certain sugars, see section 2 in this leaflet and refer to the

doctor.

How does the medicine look like and what are the contents of

the package:

Avilac is a clear viscous colorless to brownish yellow liquid, and

comes in a white or brown bottle with a white cap and contains

300 ml medicine.

Signs of overdose: You may suffer from diarrhea and abdominal

pain. In case of fluid loss due to diarrhea or vomiting, the doctor

may recommend discontinuation of the treatment or reduction

of the dosage.

If you forget to take the medicine

If you forget to take the medicine at the designated time, do not

take a double dose. Take a dose as soon as you remember, but if

it is time for the next dose, skip the forgotten dose and take the

next dose at the regular time.

How can you contribute to the success of the treatment?

Do not take medicines in the dark! Check the label and the dose

each time you take medicine. Wear glasses if you need them.

If you have further questions regarding use of this medicine, consult

the doctor or pharmacist.

4. SIDE EFFECTS

As with any medicine, use of Avilac may cause side effects in some

users. Do not be alarmed when reading the list of side effects. You

may not suffer from any of them.

Refer to the doctor as soon as possible if you are being treated

with Avilac for hepatic coma and suffer from diarrhea.

Occurring very frequently (frequency of more than 1:10):

diarrhea.

Occurring frequently (frequency of up to 1:10): flatulence, nausea,

vomiting, abdominal pain.

Occurring infrequently (frequency of up to 1:100): electrolyte

imbalance (electrolyte deficiency in the blood) caused by diarrhea.

At high dosages, particularly for treatment of hepatic coma,

During treatment it is recommended to drink lots of fluids, at least

1.5-2 liters a day (6-8 cups during the day), which will help soften

the feces.

Because of the special activity of lactulose, 48 hours may pass

before feeling the effect of the medicine.

If diarrhea occurs, lower the dosage.

In the event that there is no improvement in the constipation

after a few days of treatment with Avilac, consult the doctor.

Do not exceed the recommended dose.

medicine. This leaflet contains concise information about the

medicine. If you have further questions, refer to the doctor or

pharmacist.

You must use the medicine in the correct manner. Consult the

pharmacist if you need further information. Consult the doctor if

signs of the illness (symptoms) worsen or if they do not improve.

Avilac Syrup is a medicine that contains a laxative ingredient

called lactulose that makes the stool softer and easier to pass

by drawing water into the bowel. The ingredient is not absorbed

into your body.

1. WHAT IS THE MEDICINE INTENDED FOR?

For treatment of constipation and acute and chronic hepatic

encephalopathy (hepatic coma).

Therapeutic group: Osmotic laxatives.

Laxatives should only be used in children only when there is a

clear need and only under medical supervision.

Blood electrolyte tests should be performed in elderly and frail

patients who are being treated in a hepatic coma and who are

receiving it for over 6 months.

Chronic use of unadjusted dosages (exceeding 2-3 soft stools

per day) or misuse can lead to diarrhea and disturbance of the

electrolyte balance.

With regard to sugar content – the usual dose for treatment of

constipation should not pose a problem for diabetic patients.

However, the dose given for treatment of hepatic encephalopathy

(hepatic coma) is usually much higher and the sugar content of

the medicine should be taken into consideration when given to

diabetic patients.

Before treatment with Avilac, inform the doctor or pharmacist

if:

You are sensitive to any food or medicine.

You are suffering from unexplained abdominal pain.

You are unable to digest lactose (milk sugar).

You are suffering, or have suffered in the past, from impaired

function of the liver, from diabetes, or if you are on a galactose-

free diet.

If you are taking or have recently taken other medicines,

including non-prescription medicines and food supplements,

tell the doctor or pharmacist.

Use of Avilac and food:

Avilac can be taken with or without food without any limitation to

the type of food and drink.

2. BEFORE USING THE MEDICINE

Do not use the medicine if:

You are sensitive (allergic) to the active ingredient or to any

of the other ingredients included in the medicine (for a list of

the inactive ingredients, see section 6 in this leaflet).

You suffer from intestinal obstruction that is not normal

constipation.

You suffer from a rare hereditary problem of galactose

(galactosemia) or fructose intolerance, or malabsorption of

glucose or galactose, or Lapp-Lactase deficiency.

You have a digestive tract perforation or are at risk of a

digestive tract perforation.

If you are uncertain, check with the doctor or pharmacist before

using Avilac Syrup.

Special warnings regarding use of the medicine:

Prolonged use may cause dependency on laxatives.

Do not use this medicine frequently or for a prolonged period

without consulting the doctor.

Do not take this medicine with other laxatives, especially at the

beginning of treatment.

This preparation is only intended for constipation that persists

for a few days. Inactivity of the bowels for a day or two does

not justify its use. Similarly, do not use it for other purposes,

such as “body cleansing” and the like.

In the event that there is no improvement in the constipation

after a few days of treatment with Avilac, or the symptoms

worsen, consult the doctor.

Children:

Laxatives should only be used in children when there is a clear

need and only under medical supervision. Avilac Syrup should

only be used in children and infants according to a doctor’s

instructions since the medicine can affect the normal reflexes

for passing stool.

Pregnancy and breastfeeding:

If you are pregnant or breastfeeding, consult the doctor before

using this medicine.

Driving and use of machines:

Avilac Syrup has a negligible or no effect on the ability to drive or

to operate machines.

Important information regarding some of the ingredients of

the medicine:

This medicine may contain small amounts of lactose (milk sugar),

galactose, epilactose or fructose. If you suffer from a rare hereditary

problem of galactose (galactosemia) or fructose intolerance, or

malabsorption of glucose or galactose, or Lapp-Lactase deficiency,

do not use this medicine. If you have a known sensitivity to certain

sugars, consult the doctor before using the medicine.

3. HOW SHOULD YOU USE THE MEDICINE?

Check with the doctor or pharmacist if you are unsure.

Hepatic encephalopathy (hepatic coma)

Adults (including the elderly): Starting dose 30-50 ml, 3 times a

day. After that, the dosage should be adjusted until 2-3 soft bowel

movements a day are achieved.

Children: There is no recommended dosage for this indication.

You may use the measuring cup provided in the package.

Avilac may be taken as is or diluted in liquids.

Shake well before use.

The daily dosage of Avilac can be taken as a single dose or can

be divided into two daily doses.

If Avilac is taken once a day, it should be taken each day at a

set time, for example, at breakfast time. Swallow the medicine

immediately, without holding it in your mouth.

For liquid medicines, use the designated measuring cup or spoon,

syringe or dropper to measure the correct amount of medicine.

If a spoon or cup or another means of measuring has not been

provided in the package, consult the pharmacist. Do not use a

household teaspoon or cup to measure the amount of medicine.

Household teaspoons and cups differ in size and you may not

receive the correct amount of medicine.

Child-resistant caps have significantly reduced the number of

poisoning incidents caused by medicines each year. However, if

you have a hard time opening the package, you can refer to the

pharmacist to ask for help in removing the safety mechanism of the

cap and to switch it to a regular cap that is easier to open.

Tests and follow-up: During treatment, you may also be referred

for blood electrolyte tests. In the event that you are taking a high

dosage for treating hepatic coma over a prolonged period, it is

possible that you will suffer from electrolyte imbalance (electrolyte

deficiency in the blood) caused by diarrhea.

If you have accidentally taken too high a dosage or an overdose,

or if a child has accidentally swallowed the medicine, proceed

immediately to a hospital emergency room and bring the package

diarrhea may occur. In such a case, continue treatment and

refer to the doctor as soon as possible to adjust your dosage

so that you will have 2-3 bowel movements a day.

Additional side effects:

Side effects may occur, especially in the first few days of treatment,

such as: intestinal bloating and flatulence - an effect which

disappears after a few days.

In the event that the dosage is too high, abdominal pain, nausea,

vomiting and diarrhea may occur. In case of diarrhea, the dosage

should be reduced.

If you suffer from diarrhea, drink lots of fluids to prevent

dehydration.

These effects usually disappear within a short time following the

period of adaptation to the medicine.

4.1 Therapeutic Indications

1. Constipation.

4.2 Posology and method of Administration

The lactulose solution may be administrated diluted or undiluted. The dose should be

titrated according to the clinical response. Lactulose may be given as a single daily dose

or in two divided doses, using the measuring cup.

Each dose of lactulose should be swallowed in one and should not be kept in the mouth

for an extended period of time.

The posology should be adjusted according to the individual needs of the patient.

In case of single daily dose this should be taken at the same time, e.g. during breakfast.

During the therapy with laxatives it is recommended to drink sufficient amounts of fluids

(1.5 – 2 litres, equal to 6-8 glasses) during the day.

The measuring cup may be used.

Dosing in constipation or where a soft stool is considered of medical benefit

After a few days the starting dosage may be adjusted to the maintenance dose based

upon treatment response. Several days (2-3 days) of treatment may be needed before

treatment effect occurs.

Paediatric population

The safety and efficacy in children (newborn to 18 years of age) with HE have not been

established. No data are available.

4.3 Contraindications

Hypersensitivity to the active substance lactulose or to any of the excipients

listed in section 6.1 .

Galactosaemia

Gastro-intestinal obstruction, digestive perforation or risk of digestive perforation.

4.4 Special warnings and precautions for use

Painful abdominal symptoms of undetermined cause should be evaluated to exclude

undiagnosed perforation or obstruction or undiagnosed disease/condition that

predisposes to either before the treatment is started.

In case of insufficient therapeutic effect after several days the dose and/or additional

measures should be re-considered.

Lactulose should be administered with care to patients who are intolerant to lactose

(see section 6.1).,

The dose normally used in constipation should not pose a problem for diabetics.

The dose used in the treatment of HE is usually much higher and may need to be taken

into consideration for diabetics.

Chronic use of unadjusted doses and misuse can lead to diarrhoea and disturbance of

the electrolyte balance.

It should be taken into account that the daefecation reflex could be disturbed during the

treatment.

This product contains lactose, galactose and small amounts of fructose. Patients with rare

hereditary problems of galactose or fructose intolerance, the Lapp lactase deficiency or

glucose-galactose malabsorption should not take this medicine.

Paediatric population

Use of laxatives in children should be exceptional and under medical supervision.

4.5 Interaction with other medicinal products and other forms of interaction

No interaction studies have been performed.

4.6 Fertility, pregnancy and lactation

Pregnancy

No effects during pregnancy are anticipated since systemic exposure to lactulose is

negligible.

Avilac oral solution can be used during pregnancy.

Breast-feeding

No effects on the breastfed newborn/infant are anticipated since the systemic exposure

of the breast-feeding woman to lactulose is negligible.

Avilac can be used during breast-feeding.

Fertility

No effects are to be expected, since systemic exposure to lactulose is negligible.

4.7 Effects on ability to drive and use machines

Avilac has no or negligible influence on the ability to drive and use machines.

4.8 Undesirable effects

Flatulence may occur during the first few days of treatment. As a rule, it disappears after

a couple of days. When dosages higher than instructed are used, abdominal pain and

diarrhoea may occur. In such a case the dosage should be decreased. See also

overdose section 4.9.

If high doses (normally only associated with hepatic encephalopathy, HE) are used for

an extended period of time, the patient may experience an electrolyte imbalance due to

diarrhoea.

Dosage should then be adjusted to obtain two or three formed stools per day.

4.9 Overdose

If the dose is too high, the following may occur:

Symptom: diarrhoea, loss of electrolytes and abdominal pain.

Treatment :cessation of treatment or dose reduction . Extensive fluid loss by

diarrhoea or vomiting may require correction of electrolyte disturbances

No specific antidote. Sympatomatic treatment should be given.

PHARMACOLOGICAL PROPERTIES

5.1 Pharmacodynamic properties

Pharmacotherapeutic group: Osmotically acting laxatives ,ATC code: A 06A D11

In the colon lactulose is broken down by colonic bacteria into low-molecular organic acids.

These acids lead to a lowering of pH in the colonic lumen and via an osmotic effect to an

increase of the volume of colonic contents. These effects stimulate peristalsis of the colon

and return the consistency of the stool. The constipation is cleared and the physiological

rhythm of the colon is reinstated.

In hepatic encephalopathy (HE), the effect has been attributed to suppression of

proteolytic bacteria by an increase of acidophilic bacteria (e.g. lactobacillus), trapping of

ammonia in the ionic form by acidification of the colonic contents, catharsis due to the low

pH in the colon as well as an osmotic effect, and alteration of the bacterial nitrogen

metabolism by stimulating the bacteria to utilize ammonia for bacterial protein synthesis.

5.2 Pharmacokinetic properties

Lactulose is poorly absorbed after oral administration and it reaches the colon

unchanged. There it is metabolised by the colonic bacterial flora. Metabolism is complete

at doses up to 25-50 g or 40 - 75 ml; at higher dosages, a proportion may be excreted

unchanged.

5.3 Preclinical safety data

The results of acute, sub-chronic and chronic toxicity studies in various species indicate

that the compound has very low toxicity. The effects observed appear to be more related

to the effect of bulk in the gastrointestinal tract than to a more specific toxic activity.

In reproduction and teratology experiments in rabbits, rats or mice no adverse effects

were found.
