PER6338 Permit Version 4 Page 1 of 2 PERMIT TO ALLOW MINOR USE OF AN AGVET CHEMICAL PRODUCT FOR MONITORING OF FRUIT FLY POPULATIONS PERMIT NUMBER - PER6338 This permit is issued to the Permit Holder in response to an application granted by the APVMA under section 112 of the Agvet Codes of the jurisdictions set out below. This permit allows a person, as stipulated below, to use the product in the manner specified in this permit in the designated jurisdictions. This permit also allows any person to claim that the product can be used in the manner specified in this permit. PER6338 Permit Version 4 Page 2 of 2 CONDITIONS OF USE PRODUCT TO BE USED: DAVID GRAYS D.D.V.P. 500 INSECTICIDE Plus OTHER REGISTERED PRODUCTS Containing: 500 g/L DICHLORVOS as their only active constituent. DIRECTIONS FOR USE: SITUATION PEST INSECT RATE Mediterranean fruit fly host Species or other suitable foliage Mediterranean Fruit Fly (_Ceratitis capitata_) 1 part Dichlorvos (500 g/L products) to 50 parts liquid pheromone. CRITICAL USE COMMENTS: The trap must be clearly labelled as specified under Additional Conditions. Dichlorvos to be mixed with pheromone (Capilure) and used in fruit fly monitoring traps. Mix 1 part dichlorvos (500 g/L products) with 50 parts liquid pheromone and apply to the wick in a laboratory fume cupboard or in a well ventilated open area. Insert wick inside the trap as required and replace approximately every 11 weeks. Traps must be placed a minumum of 1.5m above the ground. WITHHOLDING PERIOD: NOT REQUIRED WHEN USED AS DIRECTED. JURISDICTION: SA only. ADDITIONAL CONDITIONS: This PERMIT provides for the use of a product in a manner other than specified on the approved label of the product.
