PER13564 Page 1 of 4 PERMIT TO ALLOW MINOR USE OF AN AGVET CHEMICAL PRODUCT FOR THE ERADICATION OF FRUIT FLY PERMIT NUMBER – PER13564 This permit is issued to the Permit Holder in response to an application granted by the APVMA under section 112 of the Agvet Codes of the jurisdictions set out below. This permit allows a person, as stipulated below, to use the product in the manner specified in this permit in the designated jurisdictions. This permit also allows any person to claim that the product can be used in the manner specified in this permit. THIS PERMIT IS IN FORCE FROM 16 AUGUST 2012 TO 30 SEPTEMBER 2017. PER13564 Page 2 of 4 CONDITIONS OF USE PRODUCTS TO BE USED: Any registered product that contains 1180 g/L maldison as its only active constituent and Any unregistered product containing 4-(p-acetoxyphenyl)-2-butanone (Cue-Lure) as its only active constituent. DIRECTIONS FOR USE: SITUATION PEST RATE Towns in Victoria subject to Queensland fruit fly Mix at a rate of 10 Queensland fruit fly litres maldison (1180 infestation g/L) with up to 25 litres Cue-Lure and 105 litres ethanol. Use this mixture to treat 5000 caneite blocks. CRITICAL USE COMMENTS: 1. Preparation of fibre boards Fibre boards (caneite) are cut into 50mm x 50mm x 12mm blocks and soaked in Cue- Lure/maldison/ethanol mixture until thoroughly saturated. Blocks are then drained for 30 minutes and allowed to dry to touch. Blocks to be packaged in appropriate leak- proof bags or containers, such as 200L drums, lockable plastic containers and sealed plastic bags. Such containers should be appropriately labelled as per Attachment 1 as to identify the contents and kept in a secure area away from children, animals, food and feedstuffs.
