PER11491 Permit Version 1 Page 1 of 3 PERMIT TO ALLOW MINOR USE OF AN AGVET CHEMICAL PRODUCT USE OF DICHLORVOS PEST STRIPS FOR FRUIT FLY MONITORING PERMIT NUMBER -PER11491 This permit is issued to the Permit Holder in response to an application granted by the APVMA under section 112 of the Agvet Codes of the jurisdictions set out below. This permit allows a person, as stipulated below, to use the product in the manner specified in this permit in the designated jurisdictions. This permit also allows any person to claim that the product can be used in the manner specified in this permit. THIS PERMIT IS IN FORCE FROM 30 JULY 2009 TO 30 JUNE 2020. PER11491 Permit Version 1 Page 2 of 3 CONDITIONS OF USE PRODUCTS TO BE USED: KILLMASTER ZERO PEST STRIP PLUS OTHER REGISTERED PRODUCTS Containing:186 g/kg DICHLORVOS as its only active constituent and formulated as pest strips (slow release generator) registered for the control of insect pests in domestic situations. DIRECTIONS FOR USE: SITUATION PURPOSE RATE/MCPHAILS TRAP Fruit fly hosts in rural and Fruit fly monitoring Apply 1 piece of pest strip urban areas (_Bactrocera _spp._)_ per trap (refer to critical use comments below). CRITICAL USE COMMENTS: Each slow release pest-strip should be cut into 9 equal pieces and place 1 piece within each McPhails Trap including an appropriate lure for the target species. Pest strips should be replaced at monthly intervals. Traps must be placed out of reach of children and at a minimum of 1.5m above the ground. Traps must be labeled in accordance with the label contained in Attachment 1. When cutting pest strips to size the user should wear protective gloves and wear a respirator or use fume hood.
