PER13583 Permit Version 2 Page 1 of 2 PERMIT TO ALLOW MINOR USE OF AN AGVET CHEMICAL PRODUCT FOR CONTROL OF BITING MIDGE LARVAE ON SANDY CANAL BEACHES IN ARTIFICIAL CANAL ESTATES PERMIT NUMBER - PER13583 This permit is issued to the Permit Holder in response to an application granted by the APVMA under section 112 of the Agvet Codes of the jurisdictions set out below. This permit allows a person, as stipulated below, to use the product in the manner specified in this permit in the designated jurisdictions. This permit also allows any person to claim that the product can be used in the manner specified in this permit. THIS PERMIT IS IN FORCE FROM 1 OCTOBER 2012 TO 30 SEPTEMBER 2018 PERMIT HOLDER: CITY OF GOLD COAST 135 Bundall Road SURFERS PARADISE QLD 4217 PERSONS WHO CAN USE THE PRODUCT UNDER THIS PERMIT: Employees of City of Gold Coast, who are trained or experienced in the use, preparation and application of agricultural chemicals. PER13583 Permit Version 2 Page 2 of 2 CONDITIONS OF USE PRODUCT TO BE USED: ALL REGISTERED PRODUCTS Containing: 500 g/L MALDISON (MALATHION) as their only active constituent. DIRECTIONS FOR USE: SITUATION INSECT PEST RATE SANDY CANAL BEACHES IN ARTIFICIAL CANAL ESTATES Biting midge larvae (_Culicoides molestus_) Apply 1 L product / ha CRITICAL USE COMMENTS: Apply by boom spray to the inter-tidal zone of the canal beach allowing a minimum of 0.5 metres from the waters edge to the nearest application point. Apply 2 hours either side of low tide. In areas where the boom spray is not appropriate or boom access is not possible, the product is to be applied by a hand held gun using a solid stream nozzle. Calibration of spray equipment is to be performed before every whole of City treatment program and records of the calibration maintained. JURISDICTION: QLD only. ADDITIONAL CONDITIONS: Users: 1.
