Što je ISMN JGL i za što se koristi?

Djelatna tvar u ISMN JGL je izosorbidmononitrat. Pripada skupini lijekova koji se nazivaju organski

nitrati. Djeluje tako da opušta stijenke krvnih žila u Vašem srcu i na taj način povećava opskrbu srca

kisikom.

ISMN JGL se koristi za profilaksu i dugotrajno liječenje poremećaja cirkulacije srčanih krvnih žila

(angine pektoris).

2.

Što morate znati prije nego počnete uzimati ISMN JGL?

Nemojte uzimati ISMN JGL:

ako ste alergični na izosorbidmononitrat, druge nitrate ili bilo koji drugi sastojak ovog lijeka

(naveden u dijelu 6.)

ako imate ozbiljno nizak krvni tlak (sistolički krvni tlak niži od 90 mmHg)

ako imate nizak tlak punjenja srca (npr. u slučaju akutnog srčanog udara)

ako imate suženje glavne arterije ili srčanih zalistaka (aortna/mitralna stenoza zalistaka)

ako imate bolest srčanog mišića sa zadebljanjem zida (hipertrofična opstruktivna kardiomiopatija)

ako je vrećica koja obavija Vaše srce (perikard) sužena i upaljena (konstriktivni perikarditis)

ako dolazi do zadržavanja tekućine u vrećici koja obavija Vaše srce (perikardijalna tamponada)

ako imate smanjeni volumen krvi (hipovolemija)

u slučaju akutnog zatajenja cirkulacije (šok, vaskularni kolaps)

u slučaju šoka kojeg uzrokuje zatajenje srca (kardiogeni šok)

u slučaju teške anemije

ako imate poremećaj s povišenim krvnim tlakom u lubanji (npr. ozljeda glave, krvarenje u mozgu)

ako imate određeni tip glaukoma (glaukom uskog kuta)

ako istovremeno koristite lijekove za liječenje erektilne disfunkcije (inhibitori 5-fosfodiesteraze),

poput sildenafila, vardenafila ili tadalafila (vidjeti dio „Drugi lijekovi i ISMN JGL“).

Ako se bilo što od navedenog odnosi na Vas, ne smijete koristiti ISMN JGL.

Upozorenja i mjere opreza

Obratite se svom liječniku ili ljekarniku prije nego uzmete ISMN JGL tablete.

Ovaj lijek nije djelotvoran kod iznenadnih napada angine pektoris. U tom slučaju treba primijeniti

brzodjelujuće nitrate.

Budite oprezni s uzimanjem ISMN JGL:

ako ste skloni problemima s cirkulacijom krvi (ortostatska disregulacija)

nakon srčanog udara

ako Vam lijeva srčana klijetka ne radi kako treba (zatajenje lijeve strane srca)

ako imate smanjenu funkciju štitnjače (hipotiroidizam)

ako imate nisku tjelesnu temperaturu (hipotermija)

ako ste pothranjeni

ako imate tešku bolest jetre ili bubrega.

Ako ste na terapiji lijekovima za liječenje glaukoma, ISMN JGL smijete koristiti samo ukoliko

redovito kontrolirate očni tlak.

Ako se bilo što od navedenog odnosi na Vas, molimo Vas obavijestite o tome svog liječnika prije

primjene lijeka ISMN JGL.

Djeca i adolescenti

Sigurnost i djelotvornost primjene lijeka ISMN JGL kod djece i adolescenata mlađih od 18 godina nije

ustanovljena.

Drugi lijekovi i ISMN JGL

Obavijestite svog liječnika ili ljekarnika ako uzimate, ste nedavno uzeli ili biste mogli uzeti bilo koje

druge lijekove.

Lijekovi za liječenje poremećaja srca i krvotoka te povišenog krvnog tlaka, npr. drugi lijekovi koji šire

krvne žile (vazodilatatori), antagonisti kalcijevih kanala, beta-blokatori, ACE inhibitori, lijekovi za

izlučivanje tekućine (diuretici), lijekovi za liječenje depresije (triciklički antidepresivi) i antipsihotici

(neuroleptici) mogu pojačati učinak sniženja krvnog tlaka lijeka ISMN JGL.

Istodobna

primjena

lijekovima

liječenje

erektilne

disfunkcije

(PDE-5-inhibitori),

npr.

sildenafilom, vardenafilom ili tadalafilom, može dovesti do jakog sniženja krvnog tlaka. Stoga se

ISMN JGL ne smije primjenjivati istovremeno s ovim lijekovima.

Učinak dihidroergotamina na povišenje krvnog tlaka (lijeka za liječenje migrene) može biti pojačan.

Acetilsalicilatna kiselina i indometacin (protuupalni analgetici) umanjuju učinak lijeka ISMN JGL.

ISMN JGL s hranom, pićem i alkoholom

Alkoholna pića mogu pojačati učinak sniženja krvnog tlaka lijeka ISMN JGL.

Trudnoća, dojenje i plodnost

Ako ste trudni ili dojite, mislite da biste mogli biti trudni ili planirate trudnoću, obratite se svom

liječniku ili ljekarniku za savjet prije nego uzmete ovaj lijek.

Ako ste trudni, ne smijete uzimati ISMN JGL, osim ako Vam liječnik nije rekao da je to neophodno.

Ako dojite, morate prestati dojiti prije nego počnete uzimati ISMN JGL.

Upravljanje vozilima i strojevima

Ovaj lijek može utjecati na Vašu sposobnost upravljanja vozilima i rada sa strojevima.

Mogu se pojaviti značajan pad krvnog tlaka, vrtoglavica i umor, naročito na početku liječenja, te ako

se istodobno konzumira alkohol. U slučaju pojave navedenih simptoma, nemojte upravljati vozilima

niti raditi sa strojevima.

3.

Kako uzimati ISMN JGL?

Uvijek uzmite ovaj lijek točno onako kako Vam je rekao liječnik. Provjerite s Vašim liječnikom ili

ljekarnikom ako niste sigurni.

Odrasli

Preporučena doza je 20 mg dva puta dnevno (što odgovara 40 mg izosorbidmononitrata na dan).

Ako je potrebno, Vaš liječnik može povisiti dozu na 20 mg tri puta dnevno (što odgovara 60 mg

izosorbidmononitrata na dan) do 40 mg dva puta dnevno (što odgovara 80 mg izosorbidmononitrata na

dan).

Maksimalna dnevna doza od 80 mg izosorbidmononitrata se ne smije prekoračiti.

Ako uzimate ISMN JGL dva puta dnevno, drugu tabletu treba uzeti ne kasnije od 8 sati nakon

primjene prve tablete.

Liječenje treba započeti s najnižom mogućom dozom te dozu polako povećavati do odgovarajuće

količine.

Stariji bolesnici (>65 godina)

Nije potrebna prilagodba doze.

Bolesnici s oštećenom funkcijom bubrega

Nisu dostupni podaci za bolesnike s oštećenom funkcijom bubrega.

Bolesnici s oštećenom funkcijom jetre

Nisu dostupni podaci za bolesnike s oštećenom funkcijom jetre

Primjena u djece i adolescenata

Ne preporučuje se primjena lijeka ISMN JGL u djece i adolescenata mlađih od 18 godina.

Način i trajanje primjene

ISMN JGL tablete uzimaju se kroz usta, nakon jela s dovoljnom količinom tekućine (npr. uz čašu

vode). Nemojte žvakati tablete. Urez služi samo kako bi se olakšalo lomljenje tablete radi lakšeg

gutanja, a ne da bi se podijelila na jednake doze.

O trajanju liječenja odlučuje liječnik. Najčešće je liječenje ovim lijekom dugotrajno.

Ako uzmete više ISMN JGL nego što ste trebali

Lezite na leđa i podignite noge na način da su iznad Vaše glave. Odmah se javite svom liječniku ili u

najbližu bolnicu.

Liječnik će odlučiti o daljnjim mjerama ovisno o težini predoziranja, odnosno tipu simptoma. Ponesite

sa sobom ovu uputu o lijeku ili pakiranje ISMN JGL kako bi liječnik znao koji lijek ste uzeli.

Može doći do glavobolje, pada krvnog tlaka, ubrzanog rada srca, umora, vrtoglavice, crvenila kože,

povraćanja i proljeva, a u slučaju teškog predoziranja do plavkasto-crvenkaste boje kože i sluznica,

ubrzanog disanja i kratkoće daha. Nakon primjene vrlo visokih doza, može doći do povišenja tlaka

unutar lubanje.

Ako ste zaboravili uzeti ISMN JGL

Nemojte uzeti dvostruku dozu kako bi nadoknadili propuštenu tabletu

.

Ako ste zaboravili uzeti Vaš lijek, uzmite lijek što prije, a sljedeću dozu uzmite u uobičajeno vrijeme.

Međutim, ukoliko je blizu vrijeme uobičajenog uzimanja tableta, ne uzimajte propuštenu dozu, već

nastavite sa sljedećom dozom u uobičajeno vrijeme.

Ako prestanete uzimati ISMN JGL

Liječenje lijekom ISMN JGL smije se promijeniti ili prekinuti isključivo nakon savjetovanja s

liječnikom.

Liječenje ovim lijekom se ne smije prekinuti naglo, nego dozu treba postepeno smanjivati

U slučaju bilo kakvih nejasnoća ili pitanja u vezi s primjenom lijeka ISMN JGL, obratite se svom

liječniku ili ljekarniku.

4.

Moguće nuspojave

Kao i svi ostali lijekovi, ovaj lijek može uzrokovati

nuspojave, iako se one neće razviti kod svakoga.

Vrlo često (mogu se javiti u više od 1 na 10 osoba):

glavobolja ("nitratska glavobolja") posebno na početku liječenja, a najčešće se povlači nakon

nekoliko dana kontinuiranog uzimanja.

Često (mogu se javiti u manje od 1 na 10 osoba):

omamljenost, lagana vrtoglavica, osjećaj slabosti

omaglica pri sjedanju ili ustajanju uzrokovana padom krvnog tlaka (ortostatska hipotenzija),

ubrzani srčani ritam (tahikardija)

crvenilo uz osjećaj vrućine.

Manje često (mogu se javiti u manje od 1 na 100 osoba):

cirkulatorni kolaps ponekad povezan s usporenim, nepravilnim otkucajima srca i nesvjesticom

(sinkopa)

mučnina, povraćanje

alergijske reakcije na koži.

Vrlo rijetko (mogu se javiti u manje od 1 na 10 000 osoba):

upalni poremećaji kože uz ljuštenje kože (eksfolijativni dermatitis).

Nepoznato (učestalost se ne može procijeniti iz dostupnih podataka):

povišene vrijednosti methemoglobina u krvi (methemoglobinemija)

pogoršanje simptoma angine pektoris (npr. jaka bol u prsištu) u slučaju pada krvnog tlaka

slaba cirkulacija (ishemija), prolazni nedostatak kisika u arterijskoj krvi (hipoksemija)

smanjena

Prijavljivanje nuspojava

Ako primijetite bilo koju nuspojavu, potrebno je obavijestiti liječnika ili ljekarnika. Ovo uključuje i

svaku moguću nuspojavu koja nije navedena u ovoj uputi. Nuspojave možete prijaviti izravno putem

nacionalnog sustava za prijavu nuspojava: navedenog u Dodatku V.

Prijavljivanjem nuspojava možete pridonijeti u procjeni sigurnosti ovog lijeka.

5.

Kako čuvati ISMN JGL?

Ovaj lijek čuvajte izvan pogleda i dohvata djece.

Čuvati u originalnom pakiranju.

Rok valjanosti otisnut je na pakiranju iza riječi EXP.

Lijek se ne smije upotrebljavati nakon isteka roka valjanosti.

Nikada nemojte nikakve lijekove bacati u otpadne vode ili kućni otpad. Pitajte svog ljekarnika kako

baciti lijekove koje više ne koristite. Ove će mjere pomoći u očuvanju okoliša

Sadržaj pakiranja i druge informacije

Što ISMN JGL sadrži?

Djelatna tvar je izosorbidmononitrat. ISMN JGL sadrži 20 mg odnosno 40 mg izosorbidmononitrata.

Druge pomoćne tvari su: celuloza mikrokristalična, natrijev škroboglikolat vrst A, silicijev dioksid;

magnezijev stearat.

Kako ISMN JGL izgleda i sadržaj pakiranja?

ISMN JGL 20 mg tablete

Tableta bijele boje, ujednačenog oblika i veličine, glatke površine i oštrih neoštećenih rubova. Jedna

strana ima urez, a na drugoj strani je utisnuto „ISMN 20“.Tablete su gorkog okusa i bez mirisa.

Urez služi samo kako bi se olakšalo lomljenje tablete radi lakšeg gutanja, a ne da bi se podijelila na

jednake doze.

ISMN JGL 40 mg tablete

Tableta bijele boje, ujednačenog oblika i veličine, glatke površine i oštrih neoštećenih rubova. Jedna

strana ima urez, a na drugoj strani je utisnuto „ISMN 40“. Tablete su gorkog okusa i bez mirisa.

Liječenje predoziranja

Uz dodatak osnovnim mjerama, poput stavljanja bolesnika u horizontalni položaj s povišenim nogama,

primjene kisika, primjene aktivnog ugljena ili lavaže želuca, potrebno je nadzirati vitalne parametre u

uvjetima intenzivne skrbi te u slučaju potrebe provesti korektivne mjere.

U slučaju teške hipotenzije i/ili šoka, treba dati nadomjestak volumena; u iznimnim slučajevima, mogu

se primijeniti noradrenalin i/ili dopamin.

Kod methemoglobinemije, ovisno o težini: primjena vitamina C, metilenskog modrila ili toluidinskog

modrila. Kod teške methemoglobinemije: liječenje kisikom, hemodijaliza, izmjena krvi.
