Classe pharmacothérapeutique : substitut du sang et fractions protéiques plasmatiques.

Indications thérapeutiques

Restauration et maintien du volume sanguin circulant lorsque l'hypovolémie a été démontrée et que l'utilisation d'un colloïde

est appropriée.

Le choix de l'albumine préférentiellement à un colloïde artificiel dépend de la situation clinique du patient et en tenant

compte des recommandations officielles.

Chez le nouveau-né et le nourrisson, ces situations cliniques sont les suivantes :

Prévention de l'ictère nucléaire du nouveau-né en cas d'hyperbilirubinémie menaçante.

Hypo-albuminémie profonde et symptomatique chez le nouveau-né et le nourrisson.

Troubles hémodynamiques du nouveau-né en cas d'hypovolémie non corrigée par le remplissage aux cristalloïdes.

Maladies congénitales de la bilirubine: hyperbilirubinémies libres menaçantes, notamment la maladie de Crigler-Najjar.

Sans objet.

Contre-indications

N'utilisez jamais VIALEBEX nouveaux-nés et nourrissons :

si vous êtes allergique à l'albumine ou à l'un des excipients contenus dans VIALEBEX.

Précautions d'emploi ; mises en garde spéciales

Faites attention avec VIALEBEX nouveaux-nés et nourrissons :

En cas d'allergie ou de réaction allergique généralisée, l'administration devra être immédiatement interrompue. En cas de

choc, le traitement symptomatique de l'état de choc doit être instauré.

Dans toutes les situations où l'hypervolémie (augmentation du volume sanguin) avec ses conséquences et l'hémodilution

(dilution du sang) peuvent représenter un risque particulier pour le patient, l'albumine doit être utilisée avec précaution. Ces

conditions sont par exemple :

insuffisance cardiaque décompensée (défaillance de la fonction cardiaque),

hypertension artérielle,

varices œsophagiennes (dilatation d'une veine de l'œsophage),

œdème pulmonaire,

syndromes hémorragiques

anémie sévère (diminution sévère du nombre de globules rouges),

anurie (absence d'émission d'urine).

Par conséquent, lors de l'administration d'albumine concentrée, il conviendra de prendre les mesures nécessaires pour

assurer une hydratation adaptée du patient qui sera surveillé avec soin afin de prévenir toute surcharge circulatoire et toute

déshydratation.

Les solutions d'albumine humaine à 200 mg/ml (20 %) sont relativement peu concentrées en électrolytes comparativement

aux solutions d'albumine humaine à 40-50 mg/ml (4-5 %). Le bilan des sels minéraux de l'organisme du patient doit être

surveillé lors de l'administration d'albumine et les mesures nécessaires doivent être prises pour restaurer ou maintenir

l'équilibre des sels minéraux.

La solution d'albumine ne doit pas être diluée avec de l'eau pour préparations injectables ce qui pourrait provoquer une

hémolyse (destruction des globules rouges) chez le patient.

Le suivi de la coagulation et de l'hématocrite (pourcentage de globules rouges dans le sang) est nécessaire lors de

l'administration de volume important. Une attention particulière sera portée afin d'assurer une substitution adaptée des autres

composants sanguins (facteurs de coagulation, sels minéraux, plaquettes et globules rouges).

Une hypervolémie (augmentation du volume circulant) peut se produire lorsque la posologie et le débit d'administration ne

sont pas adaptés à l'état circulatoire du patient. Il faut interrompre immédiatement l'injection dès l'apparition des premiers

signes de surcharge cardiovasculaire (maux de tête, difficulté respiratoire, turgescence des veines jugulaires) ou d'une

augmentation de la pression artérielle, de la pression veineuse ou d'un œdème pulmonaire.

Le risque de transmission d'agents infectieux ne peut pas être définitivement exclu lorsque sont administrés des

médicaments préparés à partir de sang ou de plasma humain. Ceci s'applique également à des agents qui pourraient

engendrer une maladie et dont la nature est jusqu'ici inconnue.

Ce risque est cependant limité par :

de stricts contrôles effectués lors de la sélection des dons par un entretien médical avec les donneurs et la réalisation de

tests de dépistage des principaux marqueurs viraux sur chaque don;

la recherche du matériel génomique du virus de l'hépatite C sur les mélanges de plasma;

le procédé de fabrication de ce produit, qui inclut des étapes d'élimination et(ou) d'inactivation des virus, dont l'efficacité a

été établie au moyen d'études de validation virale.

L'efficacité de l'élimination et(ou) de l'inactivation virale reste cependant limitée vis-à-vis de certains virus (virus non

enveloppés) particulièrement résistants.

L'albumine, fabriquée selon un procédé bien établi, conformément aux spécifications de la Pharmacopée Européenne,

présente une sécurité virale reconnue.

Aucun cas de contamination virale associée à l'administration d'albumine humaine n'a été rapporté à ce jour.

Dans l'intérêt du patient, il est recommandé, lors de chaque administration de VIALEBEX, d'enregistrer, lorsque c'est

possible, le nom et le numéro de lot du médicament reçu.

VIALEBEX nouveau-nés et nourrissons contient 28 mg de sodium par flacon de 10 ml - en tenir compte en cas de régime

restreint en sels strict - et ne contient pas plus de 200 µg/l d'aluminium.

Instructions pour un bon usage

Posologie, Mode et/ou voie(s) d'administration, Fréquence d'administration et Durée du traitement

La concentration, la posologie et la vitesse d'administration doivent être adaptées à chaque cas particulier.

Posologie

La posologie doit être adaptée à chaque cas particulier. Elle est dictée par les paramètres cliniques, en particulier l'âge et le

poids du nouveau-né ou du nourrisson, et les paramètres hémodynamiques (pression artérielle, pression veineuse centrale,

diurèse (quantité horaire d'urine émise)) et biochimiques usuels (taux plasmatiques de protéines totales ou d'albumine).

Dans le traitement des hypovolémies (diminution du volume du sang circulant), la posologie est habituellement de 1 à 2

g/kg, administrée sous forme d'albumine à 10 % (dilution au ½ dans du glucose à 5 % ou du chlorure de sodium à 0,9 %), en

fonction de la natrémie (taux de sodium dans le sang du nouveau-né ou du nourrisson).

Dans la prévention de l'ictère nucléaire du nouveau-né, la posologie est de 1 g/kg administré après dilution au ½.

Lorsqu'une hypoprotidémie (taux anormalement bas de protéines dans le sang) accompagne l'hypovolémie (diminution du

volume de sang circulant), le taux de protéines totales résultant de la perfusion doit être contrôlé.

Les paramètres hémodynamiques doivent être régulièrement contrôlés lors de l'administration d'albumine humaine,

notamment :

pression artérielle et pouls,

pression veineuse centrale,

pression artérielle pulmonaire,

diurèse (volume urinaire),

électrolytes (sels minéraux),

hématocrite / hémoglobine.

Ce médicament peut être utilisé chez les prématurés et les patients dialysés.

Si vous avez l'impression que l'effet de VIALEBEX nouveaux-nés et nourrissons est trop fort ou trop faible, consultez votre

médecin ou votre pharmacien.

Mode d'administration

La solution doit être injectée exclusivement par voie intraveineuse soit directement soit après dilution au ½ dans une

solution isotonique, p. ex: glucose à 50 mg/ml (5 %) ou chlorure de sodium à 9 mg/ml (0,9 %). La solution d'albumine ne doit

pas être diluée avec de l'eau pour préparations injectables ce qui serait à l'origine d'une hémolyse.

L'albumine humaine ne doit pas être mélangée avec d'autres médicaments (en dehors des solutions de dilutions

recommandées), avec du sang total ou des concentrés de globules rouges.

Si nécessaire la solution doit être amenée à température ambiante ou corporelle avant perfusion.

Lorsque plusieurs flacons sont injectés et(ou) lorsque la solution est diluée, utiliser une seringue électrique dont le débit sera

réglé en fonction de l'état clinique du nouveau-né ou du nourrisson, de la tolérance et de l'indication. Il est de l'ordre de 1 à 2

ml/mn.

La solution doit être limpide ou légèrement opalescente, incolore, jaune, ambre ou verte. Ne pas utiliser de solution

présentant un aspect non homogène ou contenant un dépôt. Cela peut indiquer une instabilité de la protéine ou une

concentration de la solution.

Une fois le flacon ouvert, son contenu doit être utilisé immédiatement.

Tout produit non utilisé ou déchet doit être éliminé conformément à la réglementation en vigueur.

Symptômes et instructions en cas de surdosage

Si vous avez pris plus de VIALEBEX nouveaux-nés et nourrissons que vous n'auriez dû :

Une hypervolémie (augmentation du volume de sang circulant) peut se produire lorsque la posologie et le débit

d'administration sont trop élevés. Il faut interrompre immédiatement la perfusion dès l'apparition des premiers signes

cliniques de surcharge cardiovasculaire (maux de tête, respiration difficile, augmentation de volume des grosses veines du

cou) ou d'une augmentation de la pression artérielle, de la pression veineuse centrale ou d'un œdème pulmonaire

(infiltration de liquide dans les poumons) et mettre en place une surveillance étroite des paramètres hémodynamiques.

Instructions en cas d'omission d'une ou de plusieurs doses

Si vous oubliez de prendre VIALEBEX nouveaux-nés et nourrissons :

Ne prenez pas de dose double pour compenser la dose que vous avez oublié de prendre.

4. QUELS SONT LES EFFETS INDESIRABLES EVENTUELS ?

Description des effets indésirables

Comme tous les médicaments, VIALEBEX 200 mg/ml NOUVEAUX-NES ET NOURRISSONS est susceptible d'avoir des

effets indésirables, bien que tout le monde n'y soit pas sujet.

De légères réactions telles que bouffées vasomotrices, urticaire, frissons - sensation de chaleur, nausées peuvent être

rarement observées. Ces réactions disparaissent normalement rapidement lors du ralentissement du débit de la perfusion ou

de son arrêt. Dans de très rares cas, des réactions sévères allant jusqu'à l'état de choc peuvent être observées. Dans ces

cas, l'administration doit être arrêtée et le traitement symptomatique de l'état de choc doit être instauré.

Si vous remarquez des effets indésirables non mentionnés dans cette notice, ou si certains effets indésirables deviennent

graves, veuillez en informer votre médecin ou votre pharmacien.

5. COMMENT CONSERVER VIALEBEX 200 mg/ml NOUVEAUX-NES ET NOURRISSONS, solution pour perfusion ?

Tenir hors de la portée et de la vue des enfants.

Date de péremption

Ne pas utiliser après la date de péremption figurant sur l'étiquette du flacon et la boîte.

Conditions de conservation

A conserver à une température ne dépassant pas +25°C. Ne pas congeler. Conserver le conditionnement primaire dans

l'emballage extérieur, à l'abri de la lumière.

Si nécessaire, mises en garde contre certains signes visibles de détérioration

N'utilisez pas VIALEBEX si vous constatez une solution trouble ou contenant un dépôt.

Les médicaments ne doivent pas être jetés au tout à l'égout ou avec les ordures ménagères. Demandez à votre pharmacien

ce qu'il faut faire des médicaments inutilisés. Ces mesures permettront de protéger l'environnement.

La substance active est : l'albumine humaine (200 mg/ml, soit 20%).

Un flacon de 10 ml contient 2 g d'albumine humaine.

La solution contient 200 mg/ml (soit 20 %) de protéines dont la pureté est d'au moins 95 % d'albumine humaine.

Les autres composants sont : le chlorure de sodium, le caprylate de sodium et l'eau pour préparations injectables.

The objective of this document is to review available information on the use of aminopenicillins and their beta-lactamase inhibitor combinations in veterinary medicines in the EU, their effect on the emergence of antimicrobial resistance (AMR) and the potential impact of resistance on human and animal health. The document provides information for the risk profiling, as recommended by the Antimicrobial Advice ad hoc Expert Group (AMEG) of the EMA.
