Contraindicaciones:

Hipersensibilidad a los componentes de la formulación. Iritis aguda

Glaucoma secundario a procesos inflamatorios.

Advertencias especiales y precauciones de uso:

E: categoría de riesgo C.

LM: se desconoce si se excreta por la leche materna.

Niños: aunque no se hayan realizados estudios adecuados en la población pediátrica,

se han descrito problemas relacionados con la edad.

Evaluar relación riesgo-beneficio en caso de asma bronquial.

Conjuntivitis o queratitis infecciosa aguda.

Puede complicar procesos inflamatorios oculares.

Contiene cloruro de benzalconio, evitar el contacto con lentes de contacto blandos.

Efectos indeseables:

Frecuentes: hiperemia conjuntival, visión borrosa o cambios de la visión cercana o lejana y

ardor.

Ocasionales: dolor periocular, cefaleas y opacidad del cristalino.

Raras:

diaforesis,

temblores

musculares,

náuseas,

vómitos,

diarreas,

sialorrea,

hipertensión, broncoespasmo y taquicardia.

Posología y método de administración:

Adultos y niños: instilar 1-2 gotas cada 6-8 h.

Modo de administración: Ocular.

Interacción con otros productos medicinales y otras formas de interacción:

El uso simultáneo con los alcaloides de la belladona puede interferir su acción, además, la

pilocarpina contrarresta los efectos midriáticos de estos medicamentos.

Uso en Embarazo y lactancia:

E: categoría de riesgo C.

LM: se desconoce si se excreta por la leche materna.

Efectos sobre la conducción de vehículos/maquinarias:

Tener precaución si se presenta visión borrosa o un cambio en la visión cercana o lejana,

especialmente durante la noche.

Sobredosis:

Se debe observar a los pacientas para detectar signos de toxicidad por pilocarpina (esto es,

salivación y sudoración no habituales, náuseas, vómitos y diarrea no habituales); si esto se

produce, puede ser necesaria una terapia con anticolonérgicos (como el sulfato de atropina:

0,5 a 1 mg por vía im o iv). En caso de sobredosificación accidental está indicado el lavado

ocular. Si se ingiere, inducir la tmesis o efectuar lavado gástrico.

Propiedades farmacodinámicas:

Mecanismo de acción: La pilocarpina es un fármaco con actividad, parasimpaticomimética que

estimula directamente los receptores colinérgicos. Produce contracción del músculo esfínter

del iris, dando lugar a constricción del músculo pupilar (miosis); constricción del músculo ciliar

que ocasiona aumento de la acomodación y una reducción de la presión intraocular asociada

a disminución de la resistencia al flujo de salida del humor acuoso. La pilocarpina también

puede inhibir la secreción del humor acuoso.

En el glaucoma de ángulo abierto no se conoce con precisión el mecanismo exacto mediante

el cual los miótico disminuyen la presión intraocular, sin embargo la contracción del músculo

ciliar aparentemente abre los espacios intertrabeculares y facilita el reflujo del humor acuoso.

En el glaucoma del ángulo cerrado la constricción de la pupila aparentemente atrae al iris,

alejándolo de las trabéculas, y de esta forma alivia el bloqueo de la malla trabecular.

Absorción: por vía ocular y puede absorberse sistémicamente

Inicio de la acción: se alcanza entre 1.5 y 2 horas y dura de 4 a 8 horas en la miosis, el

efecto máximo se alcanza en 75 minutos y en la reducción de la presión intraocular en

dependencia de la formulación empleada.

Eliminación: presumiblemente ocurre por la orina excretándose sin cambios o en sus

productos de degradación.

Instrucciones de uso, manipulación y destrucción del remanente no utilizable del

producto:

Mantenga el envase dentro del estuche original. Desechar al mes de abierto el envase.
