5. How to store the medicine?

Avoid poisoning! This medicine and any other

medicine should be kept in a closed place

If you accidentally have taken a higher dosage

If you have taken an overdose or if a child has

accidentally

swallowed

medicine,

refer

immediately to a physician or to the hospital

emergency room and bring the package of the

medicine with you.

If you take too much Lamictal, you are more

likely to develop serious side effects, which

may be fatal.

Someone who has taken too much Lamictal may

have any of these symptoms:

rapid,

uncontrollable

movements

)nystagmus(

clumsiness and lack of coordination, affecting

balance )ataxia(

heart rhythm changes )detected usually on

ECG(

loss of consciousness, fits )seizures( or coma.

If you forgot to take a single dose of Lamictal

→ In this case, don’t take extra tablets to

make up for a forgotten dose. Take your

next dose at the usual time.

If you forgot to take several doses of Lamictal

→ Ask your physician for advice on how to

start taking it again. It is important that you

do this.

Don’t stop taking Lamictal without advice

You must take Lamictal for as long as your

physician recommends. Don’t stop unless your

physician advises you to.

If you’re taking Lamictal to treat epilepsy

To stop taking Lamictal, it is important that

the dosage is reduced gradually, over about

2 weeks. If you suddenly stop taking Lamictal,

your epilepsy may come back or get worse.

disorder

Lamictal may take some time to work, so you are

unlikely to feel better straight away. If you stop

taking Lamictal, your dosage will not need to be

reduced gradually but you should still talk to your

physician first, if you want to stop taking Lamictal.

the label and the dose each time you take a

medicine. Wear glasses if you need them.

4. Side effects

As with any medicine, use of Lamictal may cause

side effects in some of the users. Do not be

alarmed by reading the list of side effects. You

may not experience any of them.

Potentially

life-threatening

reactions:

A small number of people taking Lamictal develop

an allergic reaction or potentially life-threatening

skin

reaction,

which

develop

into

more

serious problems if they are not treated.

These symptoms are more likely to happen during

the first few months of treatment with Lamictal,

especially if the starting dosage is too high or if

the dosage is increased too quickly or if Lamictal

is taken with another medicine called valproate.

Some of the symptoms are more common in

children, so parents should be especially careful

to watch out for them.

If you’re taking Lamictal for epilepsy

The seizures in some types of epilepsy may

occasionally

become

worse

happen

more

often while you’re taking Lamictal. Some patients

experience

severe

seizures,

which

cause serious health problems. If your seizures

happen more often or if you experience a severe

seizure while you’re taking Lamictal:

→ Refer to a physician as soon as possible.

Children and adolescents

Lamictal is not intended for children under 2 years

of age.

Lamictal is not intended for people aged under

18 years to treat bipolar disorder. Medicines

treat

depression

other

mental

health

problems increase the risk of suicidal thoughts

and behaviour in children and adolescents aged

under 18 years.

Drug interactions

If

you

are

taking,

or

have

recently

taken,

other medicines, including non-prescription

medicines and nutritional supplements, tell

the physician or the pharmacist.

Your physician needs to know if you are taking

other medicines to treat epilepsy or mental health

problems. This is to make sure you take the correct

dosage of Lamictal. These medicines include:

oxcarbazepine,

felbamate,

gabapentin,

levetiracetam,

pregabalin,

topiramate

zonisamide, used to treat epilepsy

lithium, olanzapine or aripiprazole, used to

treat mental health problems

bupropion,

used

treat

mental

health

problems or to stop smoking.

→ Tell your physician if you are taking any

of these.

Some medicines interact with Lamictal or make

it more likely that people will have side effects.

valproate, used to treat epilepsy and mental

contraceptives work, although it’s unlikely to make

them less effective. If you are using a hormonal

contraceptive and you notice any changes in

your menstrual pattern, such as breakthrough

bleeding or spotting between periods:

→ Tell your physician. These may be signs

that

Lamictal

affecting

your

contraceptive is working.

Pregnancy and breast-feeding

→ If you are pregnant, think you may be

pregnant or are planning to have a baby,

ask

your

physician

or

pharmacist

for

advice before taking this medicine.

You should not stop treatment without

discussing

this

with

your

physician.

This is particularly important if you have

epilepsy.

Pregnancy may alter the effectiveness of

Lamictal, so you may need blood tests and

your dose of Lamictal may be adjusted.

There may be a small increased risk of

birth defects, including a cleft lip or cleft

palate, if Lamictal is taken during the first

3 months of pregnancy.

Your physician may advise you to take

extra folic acid if you’re planning to become

pregnant and while you’re pregnant.

→ If you are breast-feeding, or are planning

to

breast-feed,

ask

your

physician

or

pharmacist for advice before taking this

medicine. The active ingredient of Lamictal

passes into breast milk and may affect your

baby. Your physician will discuss the risks and

benefits of breast-feeding while you’re taking

Lamictal, and will check your baby from time to

time, whether drowsiness, rash or poor weight

gain

occurs,

decide

breast-feed.

Inform your physician if you observe any of

these symptoms in your baby.

Driving and using machines

Lamictal can cause dizziness and double vision.

→ Don’t drive or use machines unless you

are sure you’re not affected.

Children should be warned against riding bicycles

or playing near roads, etc.

If you have epilepsy, talk to your physician

about driving and using machines.

Important

information

about

some

of

the

ingredients of the medicine

Lamictal Dispersible/Chewable Tablets contain

less than 1 mmol sodium )23 mg( per tablet, i.e.,

they are essentially sodium-free.

3. How should you use the medicine?

Always use the preparation according to the

physician’s instructions. You should check with

the physician or the pharmacist if you are unsure

regarding the dosage and treatment regimen of

the preparation.

The dosage and treatment will be determined

only by the attending physician on an individual

basis according to the severity of the disease.

How much Lamictal to take

It may take a while to find the best dosage

of Lamictal for you. The dosage you take will

depend on:

your age

whether you are taking Lamictal with other

medicines

whether

have

kidney

liver

problems.

Your physician will prescribe for you a low dosage

to start and gradually increase the dosage over a

few weeks until you reach a dosage that works

for you )called the effective dosage(. Never take

more Lamictal than your physician tells you to.

Do not exceed the recommended dose

How to take your dose of Lamictal

Take your dose of Lamictal once or twice a day,

as your physician instructed you. It can be taken

with or without food.

Always take the full dose that your physician

has prescribed for you. Never take only part of

a tablet.

Your physician may also advise you to start or

stop taking other medicines, depending on what

condition you’re being treated for and the way

you respond to treatment.

Lamictal Dispersible/Chewable Tablets can be

swallowed whole with a little water or chewed or

mixed with water to make a liquid medicine.

To chew the tablet:

You may need to drink a little water while chewing

the tablet to help the tablet dissolve in the mouth.

Then drink some more water to make sure all the

medicine has been swallowed.

To make a liquid medicine:

Put the tablet in a glass with at least enough

water to cover the entire tablet.

Either stir to dissolve or wait until the tablet is

fully dissolved.

Drink all the liquid.

Add a little more water to the glass and drink it

to make sure no medicine is left in the glass.

Anti-epileptic medicines are used to treat several

conditions,

People

bipolar

disorder

sometimes have suicidal thoughts or thoughts of

harming themselves. If you have bipolar disorder,

you may be more likely to think like this:

when you first start treatment or with a change

in dosage

if you previously had thoughts about harming

yourself or about suicide

if you are under 25 years old.

If you have distressing thoughts or experiences,

or if you notice that you feel worse or develop

new symptoms while you’re taking Lamictal:

→ Refer to a physician as soon as possible or

go to the nearest hospital for help.

You may find it helpful to tell a family

member, caretaker or a close friend that

you

can

become

depressed

or

have

significant

mood

swings

and

ask

them

to read this leaflet. You can ask them to

tell you if they are concerned about your

depression or about other changes in your

behavior.

A small number of people being treated with

anti-epileptics such as Lamictal have also had

thoughts of harming or killing themselves. If at

any time you have these thoughts, immediately

contact your physician.

Lamictal Dispersible/Chewable Tablets 2mg are white round to off-white tablets with a blackcurrant odour.

One side has a bevelled edge and is marked LTG above the number 2. The other side is marked with two

overlaping super-ellipses at right angles. The tablets may be slightly mottled.

Lamictal Dispersible/Chewable Tablets 5mg are white to off-white, elongated, biconvex, tablets, with a

blackcurrant odour, marked “GS CL2” on one side and “5” on the other. The tablets may be slightly

mottled.

Lamictal Dispersible/Chewable Tablets 25mg are white to off-white, multifaceted, super epileptical tablets

with a blackcurrant odour, marked “GSCL5” on one side“25” on the other. The tablets may be slightly

CLINICAL PARTICULARS

4.1

Therapeutic indications

Epilepsy

(Lamictal Disp/Chewable Tablets : 2MG, 5MG, 25MG, 50MG, 100MG, 200MG)

Adults and adolescents aged 13 years and above

Adjunctive or monotherapy treatment of partial seizures and generalised seizures, including

tonic-clonic seizures.

Seizures associated with Lennox-Gastaut syndrome. Lamictal is given as adjunctive therapy but may

be the initial antiepileptic drug (AED) to start with in Lennox-Gastaut syndrome.

Children and adolescents aged 2 to 12 years

Adjunctive treatment of partial seizures and generalised seizures, including tonic-clonic seizures and

the seizures associated with Lennox-Gastaut syndrome.

Monotherapy of

typical absence seizures.

Bipolar disorder

(Lamictal Disp/Chewable Tablets : 25MG, 50MG, 100MG, 200MG)

Adults aged 18 years and above

Prevention of depressive episodes in patients with bipolar I disorder who experience predominantly

depressive episodes (see section 5.1).

Lamictal is not indicated for the acute treatment of manic or depressive episodes.

4.2

Posology and method of administration

Lamictal chewable/dispersible tablets may be chewed, dispersed in a small volume of water (at least enough

to cover the whole tablet) or swallowed whole with a little water

If the calculated dose of lamotrigine (for example for treatment of children with epilepsy or patients with

hepatic impairment) does not equate to whole tablets, the dose to be administered is that equal to the lower

number of whole tablets.

Restarting therapy

Prescribers should assess the need for escalation to maintenance dose when restarting Lamictal in patients

who have discontinued Lamictal for any reason, since the risk of serious rash is associated with high initial

doses and exceeding the recommended dose escalation for lamotrigine (see section 4.4). The greater the

interval of time since the previous dose, the more consideration should be given to escalation to the

maintenance dose. When the interval since discontinuing lamotrigine exceeds five half-lives (see

section 5.2),

Lamictal should generally be escalated to the maintenance dose according to the appropriate

schedule.

It is recommended that Lamictal not be restarted in patients who have discontinued due to rash associated

with prior treatment with lamotrigine unless the potential benefit clearly outweighs the risk.

Epilepsy

The recommended dose escalation and maintenance doses for adults and adolescents aged 13 years and

above (Table 1) and for children and adolescents aged 2 to 12 years (Table 2) are given below. Because of a

risk of rash the initial dose and subsequent dose escalation should not be exceeded (see section 4.4).

When concomitant AEDs are withdrawn or other AEDs/medicinal products are added on to treatment

regimes containing lamotrigine, consideration should be given to the effect this may have on lamotrigine

pharmacokinetics (see section 4.5).

2 mg chewable/dispersible tablets may be taken on alternate days for the first two weeks. If the calculated

daily dose in patients taking valproate is less than 1 mg, then Lamictal should not be administered.

To ensure a therapeutic dose is maintained the weight of a child must be monitored and the dose reviewed as

weight changes occur. It is likely that patients aged two to six years will require a maintenance dose at the

higher end of the recommended range.

If epileptic control is achieved with adjunctive treatment, concomitant AEDs may be withdrawn and patients

continued on Lamictal monotherapy

Children below 2 years

There are limited data on the efficacy and safety of lamotrigine for adjunctive therapy of partial seizures in

children aged 1 month to 2 years (see section 4.4). There are no data in children below 1 month of age. Thus

Lamictal is not recommended for use in children below 2 years of age. If, based on clinical need, a decision

to treat is nevertheless taken, see sections 4.4, 5.1 and 5.2.

Bipolar disorder

The recommended dose escalation and maintenance doses for adults of 18 years of age and above are given

in the tables below. The transition regimen involves escalating the dose of lamotrigine to a maintenance

stabilisation dose over six weeks (Table 3) after which other psychotropic medicinal products and/or AEDs

can be withdrawn, if clinically indicated (Table 4). The dose adjustments following addition of other

psychotropic medicinal products and/or AEDs are also provided below (Table 5). Because of the risk of rash

the initial dose and subsequent dose escalation should not be exceeded (see section 4.4).

Discontinuation of Lamictal in patients with bipolar disorder

In clinical trials, there was no increase in the incidence, severity or type of adverse reactions following

abrupt termination of lamotrigine versus placebo. Therefore, patients may terminate Lamictal without a

step-wise reduction of dose.

Children and adolescents below 18 years

Lamictal is not recommended for use in children below 18 years of age because a randomised withdrawal

study demonstrated no significant efficacy and showed increased reporting of suicidality

(see section 4.4 and

5.1).

General dosing recommendations for Lamictal in special patient populations

Women taking hormonal contraceptives

The use of an ethinyloestradiol/levonorgestrel (30 μg/150 μg) combination increases the clearance of

lamotrigine by approximately two-fold, resulting in decreased lamotrigine levels. Following titration, higher

maintenance doses of lamotrigine (by as much as two-fold) may be needed to attain a maximal therapeutic

response. During the pill-free week, a two-fold increase in lamotrigine levels has been observed.

Dose-related adverse events cannot be excluded. Therefore, consideration should be given to using

contraception without a pill-free week, as first-line therapy (for example, continuous hormonal

contraceptives or non-hormonal methods; see sections 4.4 and 4.5).

Starting hormonal contraceptives in patients already taking maintenance doses of lamotrigine and NOT

taking inducers of lamotrigine glucuronidation

The maintenance dose of lamotrigine will in most cases need to be increased by as much as two-fold (see

sections 4.4 and 4.5). It is recommended that from the time that the hormonal contraceptive is started, the

lamotrigine dose is increased by 50 to 100 mg/day every week, according to the individual clinical response.

Dose increases should not exceed this rate, unless the clinical response supports larger increases.

Measurement of serum lamotrigine concentrations before and after starting hormonal contraceptives may be

considered, as confirmation that the baseline concentration of lamotrigine is being maintained. If necessary,

the dose should be adapted. In women taking a hormonal contraceptive that includes one week of inactive

treatment ("pill-free week"), serum lamotrigine level monitoring should be conducted during week 3 of

active treatment, i.e. on days 15 to 21 of the pill cycle. Therefore, consideration should be given to using

contraception without a pill-free week, as first-line therapy (for example, continuous hormonal

contraceptives or non-hormonal methods; see sections 4.4 and 4.5).

Stopping hormonal contraceptives in patients already taking maintenance doses of lamotrigine and NOT

taking inducers of lamotrigine glucuronidation

The maintenance dose of lamotrigine will in most cases need to be decreased by as much as 50% (see

sections 4.4 and 4.5). It is recommended to gradually decrease the daily dose of lamotrigine by 50-100 mg

each week (at a rate not exceeding 25% of the total daily dose per week) over a period of 3 weeks, unless the

clinical response indicates otherwise. Measurement of serum lamotrigine concentrations before and after

stopping hormonal contraceptives may be considered, as confirmation that the baseline concentration of

lamotrigine is being maintained. In women who wish to stop taking a hormonal contraceptive that includes

one week of inactive treatment ("pill-free week"), serum lamotrigine level monitoring should be conducted

during week 3 of active treatment, i.e. on days 15 to 21 of the pill cycle. Samples for assessment of

lamotrigine levels after permanently stopping the contraceptive pill should not be collected during the first

week after stopping the pill.

Starting lamotrigine in patients already taking hormonal contraceptives

Dose escalation should follow the normal dose recommendation described in the tables.

Starting and stopping hormonal contraceptives in patients already taking maintenance doses of lamotrigine

and TAKING inducers of lamotrigine glucuronidation

Adjustment to the recommended maintenance dose of lamotrigine may not be required.

Use with atazanavir/ritonavir

No adjustments to the recommended dose escalation of lamotrigine should be necessary when lamotrigine is

added to the existing atazanavir/ritonavir therapy.

In patients already taking maintenance doses of lamotrigine and not taking glucuronidation inducers, the

lamotrigine dose may need to be increased if atazanavir/ritonavir is added, or decreased if

atazanavir/ritonavir is discontinued. Plasma lamotrigine monitoring should be conducted before and during 2

weeks after starting or stopping atazanavir/ritonavir, in order to see if lamotrigine dose adjustment is needed

(see section 4.5).

Use with lopinavir/ritonavir

No adjustments to the recommended dose escalation of lamotrigine should be necessary when lamotrigine is

added to the existing lopinavir/ritonavir therapy.

In patients already taking maintenance doses of lamotrigine and not taking glucuronidation inducers, the

lamotrigine dose may need to be increased if lopinavir/ritonavir is added, or decreased if lopinavir/ritonavir

is discontinued. Plasma lamotrigine monitoring should be conducted before and during 2 weeks after starting

or stopping lopinavir/ritonavir, in order to see if lamotrigine dose adjustment is needed (see section 4.5).
