Propentofyllin

Eigenschaften / Wirkungen

Propentofyllin gehört zur Gruppe der Xanthinderivate. Es fördert die Durchblutung im

Bereich der Mikrozirkulation. Arterielle Durchblutungsstörungen im Bereich der

Mikrozirkulation sind mit einer Verschlechterung der Fliesseigenschaften (Blutrheologie)

der Erythrozyten verbunden.

In vitro verbessert Propentofyllin die Verformbarkeit experimentell geschädigter roter

Blutkörperchen und erhöht dadurch die Fliessrate gealterter und Kalzium-geschädigter

Erythrozyten.

Durchblutungsstörungen gehen häufig mit einer gesteigerten Aggregation der

Blutplättchen einher. In vitro Versuche mit Kaninchen- und Hundeblut ergaben eine

Hemmung der experimentell induzierten Thrombozytenaggregation. Abhängig von den

Krankheitserscheinungen kann die Wirkung schon nach wenigen Tagen einsetzen. Nach

Absetzen der Behandlung ist mit einer Wiederkehr der Symptome zu rechnen.

Pharmakokinetik

Propentofyllin wird nach oraler Applikation rasch und vollständig resorbiert und wie nach

intravenöser Gabe schnell im Gewebe verteilt. Nach oraler Gabe beim Hund werden

maximale Spiegel im Plasma schon nach 15 Minuten erreicht. Die Halbwertzeit beträgt

etwa 30 Minuten und die Bioverfügbarkeit für die Ausgangssubstanz ca. 30%.

Propentofyllin wird zu einer Reihe von wirksamen Metaboliten verstoffwechselt, die

massgeblich zur pharmakologischen Wirkung beitragen. Die Biotransformation erfolgt

vorwiegend in der Leber und die Elimination in Form von Metaboliten erfolgt zu 80 - 90%

über die Niere. Der Rest wird mit dem Kot ausgeschieden. Es kommt zu keiner

Kumulation.

Indikationen

Altersbeschwerden beim Hund (Durchblutungsstörungen im zerebralen und peripheren

Bereich).

Die Tagesdosis beträgt 6 - 10 mg Propentofyllin / kg Körpergewicht. Diese sollte verteilt

auf zwei Einzelgaben von jeweils 3 - 5 mg/kg Körpergewicht wie folgt verabreicht werden:

Eine exaktere Dosierung kann durch die Gabe von Vierteln der 100 mg Tablette erzielt

werden. Die Kautabletten können direkt eingegeben oder in einer kleinen Menge Futter

vermischt gegeben werden, sollten aber mindestens 30 Minuten vor der eigentlichen

Fütterung verabreicht werden.

Anwendungseinschränkungen

Kontraindikationen

Nicht anwenden bei Hunden mit einem Körpergewicht unter 5 kg.

Aufgrund fehlender Untersuchungen nicht bei trächtigen Hündinnen und Zuchttieren

anwenden.

Vorsichtsmassnahmen

Da die Kautabletten aromatisiert sind, besteht die Gefahr, dass Hunde diese gezielt

suchen und übermässig aufnehmen. Propentotab Kautabletten müssen deshalb

ausserhalb der Reichweite der Tiere aufbewahrt werden.

Bei deutlich verminderter Nierenfunktion ist die Dosis zu reduzieren.

Unerwünschte Wirkungen

VetVigilance: Pharmacovigilance-Meldung erstatten

Selten können nach Gabe von Propentotab allergische Hautreaktionen (z.B. Urtikaria)

auftreten, die ein Absetzen der Behandlung erforderlich machen.

In seltenen Fällen wurde Erbrechen beobachtet, insbesondere zu Beginn der Therapie.

Anzeichen einer kardialen und zerebralen Überstimulation wurden beobachtet. In solchen

Fällen sollten die Tiere symptomatisch behandelt werden.

Keine bekannt.

Sonstige Hinweise

Arzneimittel für Kinder unerreichbar aufbewahren.

Unterhalb von 30°C lagern. Haltbarkeit von Propentotab 100 mg Tablettenteilen im Blister:

4 Tage.

Das Präparat darf nur bis dem auf dem Behälter mit 'EXP' bezeichneten Datum verwendet

werden.
