Dette er et sammendrag af den europæiske offentlige vurderingsrapport (EPAR) for Insuman. Det

forklarer, hvordan Udvalget for Lægemidler til Mennesker (CHMP) vurderede lægemidlet og nåede frem

til sin udtalelse til fordel for udstedelse af en markedsføringstilladelse og til sine anbefalinger om

anvendelsesbetingelserne for Insuman.

Hvad er Insuman?

Insuman er en række forskellige opløsninger og suspensioner af insulin til injektion. Insuman leveres i

hætteglas, cylinderampuller og færdigfyldte injektionspenne til engangsbrug. Insuman-produkterne er

følgende:

Insuman Rapid, Insuman Infusat og Insuman Implantable, der er hurtigtvirkende

insulinopløsninger, der indeholder opløselig insulin

Insuman Basal, en insulinsuspension med mellemlang virkningstid, som indeholder isophaninsulin

Insuman Comb, som fås i forskellige kombinationer af insuliner med hurtig eller mellemlang

virkningstid.

Hvad anvendes Insuman til?

Insuman anvendes hos patienter med diabetes (type 1 og 2), som behøver behandling med insulin.

Insuman Rapid kan desuden anvendes til behandling af hyperglykæmisk koma (bevidstløshed

forårsaget af for højt blodsukker) og ketoacidose (høje koncentrationer af ketoner [syrer] i blodet),

samt til regulering af blodsukkeret før, under og efter en operation.

Insuman Implantable anvendes hos patienter med type 1-diabetes, der ikke kan kontrolleres med

insulin givet under huden, og som ofte oplever episoder af, i øvrigt uforklarlige, hyper- eller

hypoglykæmi (for højt eller for lavt blodsukkerniveau).

Hvordan anvendes Insuman?

Insuman Rapid, Infusat, Basal og Comb gives ved injektion under huden, generelt i bugvæggen

(maven) eller låret ifølge lægens anbefalinger. Injektionsstedet ændres for hver injektion. Det ønskede

blodsukkerniveau og Insuman-produkt samt dosering og tidsplan for injektionerne fastlægges af lægen

for hver patient individuelt og tilpasses dennes diæt, fysiske aktiviteter og livsstil. Patientens blodsukker

bør kontrolleres regelmæssigt for at fastsætte den effektive minimumsdosis. Insuman bør gives før

måltiderne. Se indlægssedlen for den præcise tidsplan.

Insuman Rapid kan også gives i en blodåre, men kun på en intensivafdeling eller tilsvarende facilitet,

hvor patienten kan blive nøje overvåget. Insuman Infusat er fremstillet specielt til anvendelse ved

kontinuerlig infusion under huden ved hjælp af en ekstern transportabel insulinpumpe.

Insuman Implantable gives kun ved infusion i bughulen ved hjælp af en pumpe (Medtronic MiniMed

implantable Pump), der implanteres under huden i maven. Insuman Implantable må ikke anvendes på

nogen anden måde og kan kun anvendes på centre, hvor personalet har fået tilstrækkelig undervisning

i brug af den implantable pumpe.

Hvordan virker Insuman?

Ved sukkersyge producerer kroppen enten ikke nok insulin til at regulere blodsukkeret (type 1-

diabetes), eller også er den ikke i stand til at udnytte insulinet effektivt (type 2-diabetes). Insuman er

en insulinerstatning, der svarer til det insulin, der produceres af kroppen.

Det aktive stof i Insuman, human insulin, fremstilles ifølge en metode, der er kendt som "rekombinant

dna-teknologi": Det fremstilles af bakterier, der har fået indsat et gen (DNA), som gør dem i stand til at

producere insulin. Insuman indeholder forskellige former for insulin: den opløselige form, der er

hurtigtvirkende (inden for 30 minutter efter injektionen), og isophan- og krystallinske protaminformer,

der optages meget langsommere i løbet af dagen og dermed virker længere.

Erstatningsinsulinet fungerer på samme måde som naturligt produceret insulin og hjælper glukosen til

at trænge ind i cellerne fra blodet. Ved at holde blodsukkerniveauet under kontrol mindskes

diabetessymptomerne og -komplikationerne.

Hvordan blev Insuman undersøgt?

Insuman blev undersøgt i tre undersøgelser hos 780 patienter med enten type 1-diabetes eller type 2-

diabetes. I en af undersøgelserne blev Insuman givet til patienter med type 1-diabetes ved hjælp af en

ekstern insulinpumpe. I en anden undersøgelse blev Insuman Comb 25 sammenlignet med

semisyntetisk human insulin hos patienter med type 1- og type 2-diabetes. Derudover blev Insuman

Implantable undersøgt hos voksne patienter med type 1-diabetes. Disse patienter fik Insuman

Implantable som en kontinuerlig infusion i bughindehulen.

Det primære effektmål i alle undersøgelserne var ændringen i koncentrationen af glykosyleret

hæmoglobin (HbA1c), som er den procentdel af hæmoglobin i blodet, hvortil der er bundet glykose.

Mængden af HbA1c er et mål for, hvor godt blodsukkeret er reguleret.

Hvilken fordel viser undersøgelserne, at der er ved Insuman?

Insuman har bevirket et fald i indholdet af HbA1c som tegn på, at blodsukkeret er blevet reguleret til

et niveau svarende til det, der opnås med semisyntetisk humant insulin. Insuman var effektivt til

behandling af såvel type 1- som type 2-diabetes.

Hvilken risiko er der forbundet med Insuman?

Insuman kan forårsage hypoglykæmi. Den fuldstændige liste over alle de indberettede bivirkninger ved

Insuman fremgår af indlægssedlen.

Insuman må ikke anvendes hos personer, som er overfølsomme (allergiske) over for humant insulin

eller andre af indholdsstofferne. Endvidere må Insuman Implantable ikke anvendes på nogen anden

måde end som kontinuerlig infusion ved hjælp af Medtronic MiniMed Implantable Pump. Det må heller

ikke anvendes hos patienter, som er overfølsomme over for titanlegeringer, polysulfon eller

siliconemateriale anvendt i komponenter af den implanterede pumpe. Der må ikke anvendes nogen

andre former for insulin med pumpen, og denne må ikke anvendes hos børn, der endnu ikke er på

størrelse med en voksen. Pumpen må ikke implanteres i personer, der er fastboende i en højde på over

2 439 meter (8000 fod).

Hvorfor blev Insuman godkendt?

CHMP besluttede, at fordelene ved Insuman opvejer risiciene ved behandling af type 1- og type 2-

diabetes. Udvalget anbefalede udstedelse af markedsføringstilladelse for Insuman.

Europa-Kommissionen udstedte en markedsføringstilladelse med gyldighed i hele Den Europæiske

Union for Insuman den 21. februar 1997.

The objective of this document is to review available information on the use of aminopenicillins and their beta-lactamase inhibitor combinations in veterinary medicines in the EU, their effect on the emergence of antimicrobial resistance (AMR) and the potential impact of resistance on human and animal health. The document provides information for the risk profiling, as recommended by the Antimicrobial Advice ad hoc Expert Group (AMEG) of the EMA.
